Description
Medical Supplies - Potential Sources Sought Military Hospitals: (Army, Navy, Air Force) Southwest Region IV START KITS: Standardization Point of Contact: Cindy Ingrao, Clinical Consultant, Tri-Service Regional Business Office, Southwest Region, San Diego, CA. Telephone: (619) 532-8517. Potential sources sought to participate in the regional standardization of IV Start Kits. Southwest Region, a military integrated delivery network (IDN), comprised of Army, Navy and Air Force medical treatment centers in California are in the process of standardizing IV Start Kits. If your company currently holds or is in the process of obtaining a Distribution and Pricing Agreement (DAPA) with the Defense Supply Center-Philadelphia (DSCP) on IV Start Kits, your company is eligible to participate in the standardization process. If your company is not a current DAPA holder for this product line, you are still eligible to participate in the standardization process by contacting the Defense Supply Center, Philadelphia (DSCP), Mr. John Charalabidis at (215) 737-7124 or e-mail at john.charalabidis@dla.mil to apply for a DAPA. Your company can be considered once DAPA paperwork is pending at DSCP. Please respond by Email: cingrao@nmcsd.med.navy.mil and provide your company name, address, point of contact, telephone, fax and E-mail address and answers to the below listed questions no later than June 24, 2005. For questions, please call (619) 532-8517. . 1.Does your company manufacture or distribute a standard IV Start Kit with Chloraprep skin prep? 2.Are your kits available through the Prime Vendor, Owens & Minor? 3.What is your DAPA number? 4.Under what brand(s) does your company manufacture or distribute your kit? 5.Under what standards is your kit manufactured? 6.Are your kits latex free? If not, are they clearly labeled as to their latex content? 7.What type of adhesive does your company use for this kit (self stick, pressure sensitive, etc)? 8.Is your adhesive non-allergenic? 9.Is your adhesive recommended for use with all skin types and populations? 10.Does your adhesive stay in place with various degrees of skin moisture (dry to diaphoretic)? 11.Can the contents of your kit be customized? 12.Can the brand of transparent dressing be customized? 13.How is your kit packaged? 14.Is your kit sterile or non-sterile? 15.Is the use and function of the kit easily identifiable on the packaging? 16.Are numbers and characters on the kit readable? 17.Does this kit require in-service training? If so, is this training provided by your company at no charge? 18.Does your company provide customer service 24/7? 19.Is there any history of backorders and/or recalls for this product group?
