- This is published as a sole source announcement under the authority of the test program for commercial items (Section 4202 of the Clinger-Cohen Act of 1996). The proposed contract action is for commercial services for which the Government intends to solicit and negotiate with only one source pursuant to the authority FAR 13.501(a)(1)(ii). See Note 22. The NAICS code for this action is 621111. The Fleet and Industrial Supply Center (FISC) San Diego, CA intends to award a sole source contract to St. Jude Medical Center. Aforementioned supplies is for the Naval Medical Center, San Diego, CA., Cardiology Department. A listing of the line item numbers and descriptions are as follows: DEFIBRILLATORS: (a) Model: Atlas Plus DR, dual chamber ICD, Model# V-243; (b) Model: Epic Plus VR, single chamber ICD, Model# V-196; (c) Model: Atlas Plus VR, single chamber ICD, Model# V-193M; DEFIBRILLATOR LEADS: (a) Model: Riata dual coil ICD lead, ICD lead wire, Model# 1580; (b) Riata dual coil ICD lead, ICD lead wire, Model# 1581; PACEMAKERS: (a) Model: Idenlity Adx XL DR, dual chamber, Model# 5386; (b) Model: Verity Adx XL DR, dual chamber, Model#5356; (c) Model: Idenlity Adx SR, single chamber, Model# 5180; (d) Model: Verity Adx SR, single chamber, Model# 5156; and PACEMAKER LEADS: Model: Tendril SDX, pacemaker lead wire, Model# 1688. The proposed contract is for a base period of one year, from 01 October 2005 through 30 September 2006, with 2 one-year option periods to follow. Interested vendors may identify their interest and capability to respond to the requirement or submit proposals. The provision at FAR 52.212-3, Offeror Representations And Certifications ? Commercial Items, (January 2004) Alternate I (April 2002) applies. This provision is a fill-in and Offerors must complete all applicable portions and submit the completed copy with its quote. To obtain a copy, please contact Ms. Evelyn Atienza. NOTE: Vendor is required to use the web-based Online Representation and Certifications Application (ORCA) for this solicitation. If you are currently registered in the Central Contractor Registration (CCR), access your account in CCR, and enter a Marketing Partner ID Number (MPIN). Then login to ORCA to add your ORCA record. The ORCA website is http://orca.bpn.gov/login.aspx. Your MPIN takes 24 to ! 48 hours, after your registration in CCR, to be activated in ORCA, so you are encouraged to complete this entire process in sufficient time so that your proposal can be submitted on time. This notice of intent is not a request for competitive proposals. However, the Government will consider all statements of interest and capability received no later than 20 June 2005. A determination by the Government not to compete the proposed contract based on responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. All statements of interest/capability documentation must be submitted via email attachment or fax to the attention of Ms. Evelyn Atienza at 619-532-4957. Mailed documents will not be accepted.
