Description: This is a SOURCES SOUGHT NOTICE to determine the availability to provide Ambulance services. Potential sources shall furnish all personnel, labor, uniforms, training, materials, equipment, vehicles, supervision and other items or services required to manage and provide ground emergency ambulance services and routine patient transportation services on a 24-hour per day basis, as required for the service area of Bayne Jones Army Community Hospital (BJACH), an Army Medical Treatment Facility (MTF) located at Fort Polk, Louisiana. This service shall be provided in accordance with Louisiana State Laws, Federal Laws, Army Regulations and the terms of this contract. Potential sources shall provide ambulance personnel currently certified by the National Registry of EMTS and registered as Emergency Medical Technicians-Basic or higher. All EMT-Basic and Intermediate shall have a current certification in Basic Life Suppo rt by the American Heart Association. All EMT-Paramedics shall have a current certification in both Basic Life Support and advance Cardiac Life Support (ACLS) by the American Heart Association. Potential sources shall provide a supervisory Contract Manager. The Contract Manager shall be the primary on site representative of the Contractor and shall be the central point of contact between the Government and the Contractor. The Contract Manager s hall not be considered a part of the minimum staffing requirement. The Contract Manager, or substitute Contract Manager designated in writing to act in the Contractors behalf, shall be available by telephone during normal working hours and present on-site a minimum of twenty hours per week for coordination with the Contracting Officer Representative (COR), and to attend necessary meetings. Additional on-site hours may be necessary at the discretion of the COR. The Contract Manager shall have daily liaiso n with the hospitals EMS leadership. During other than normal working hours, the Emergency Medical Service (EMS) Shift Leader shall be a working assistant to the Contract Manager performing supervisory functions to the Contractors employees. The Contra ctor shall provide a Contract Manager and/or substitute contract Manager with the following qualifications: The Contract Manager and substitute Contract Manager shall maintain certification/registration, during his/her employment under this contract, by t he National Registry of Emergency Medical Technicians (NREMTs) and registration as an Emergency Medical Technicians (EMT) or higher level and certification in Basic Life Support (BLS) by the American Heart Association. The Contract Manager and substitute Contract Manager shall have a minimum of five years of experience as an EMT-Basic (EMT-B) or higher, of which two years experience shall have been of active service as EMT-Basic or higher within the last three years. Potential sources shall provide a ground dispatcher and a minimum of one Advance Life Support (ALS) unit fully staffed 24 hours a day, 7 days a week, 365 days a year and one additional ALS unit fully staffed 12 hours a day, 7 days a week, 365 days a year f or emergency and routine transports. The ALS units shall be staffed with a minimum of one EMT-Paramedic and one EMT-Basic or higher. Personnel shall not work more than 16 hours within a 24-hour time period. Potential sources shall supply additional EMTs whenever a long distance critical transfer or other emergency situation shall arise and an ambulance and two EMTs are anticipated to be unavailable for more than two hours. A minimum of one EMT (nation ally certified Basic or higher) shall accompany each routine dispatch. Sufficient EMTs to support routine patient transport shall be provided as required. Potential sources shall provide a vehicle for emergency care which meets federal standards; can accommodate two EMTs and two litter patients, positioned so that the patient can be given intensive life support during transit; carries equipment and medical s upplies for optimal care at the scene and during transport; has two-way radio communication; is designed for light rescue procedures; is constructed to afford maximum patient safety and comfort; and avoids aggravation of the patients condition, exposure t o complications, and threat to survival. Potential sources ambulance personnel shall have a valid driver license in accordance with Louisiana law. Potential sources shall be in compliance with LRS 40:1235 et al and licensed by the State in accordance with LRS 40: 1235.2. Potential sources shall provide a yearly Emergency Driver Course for all personnel assigned to the Ambulance Section. Ambulance personnel shall have their civilian drivers licenses on their persons while driving a vehicle in the performance of services under this contract. Potential sources shall service the area for emergency ambulance service (initial response) is the geographic area within the boundaries of Fort Polk Installation to include Sycamore Point Housing Area and Timber Ridge Housing Area. This is not a Request For Proposal (RFP). There is no obligation on behalf of the government to issue a solicitation as a result of this market survey.
