Description
This is a market survey and sources sought synopsis to identify qualified small businesses to provide wigs for chemotherapy patients at the National Institutes of Health, Warren Grant Magnuson Clinical Center. The primary mission of the Wig Service Program is to supply individualized service to the needs of male and female, adult and pediatric patients who suffer hair loss due to chemotherapy. The goal of the mission is to serve the patients satisfactorily in terms of wig quality, texture, color, style, fit, care, service, delivery, and respect for privacy. The Standard Industrial Classification Code for the requirement is 541890. The NIH Warren Grant Magnuson Clinical Center has approximately 10 percent of the patients requiring wigs, male and female, elementary school-age children and teenagers. The remaining 90% will be adult male and female patients whose ages may range from 18 to 70 years. The census usually shows a 50/50 distribution of male and female patients. The distribution of wigs in the program is generally 5% male and 95% female. In submission of the capability statements, address the following questions: 1)Describe your shop area in reference to the facilities in providing sufficient booths/rooms for individualized patient privacy in fitting and styling; 2) What is the inventory of wigs available for male or female, adults or children of all ethnic groups; 3) Describe staffing and qualifications in exhibiting ability or willingness to work with seriously ill patients; 4) What are the quality and durability of the wigs; and 5) Pricing arrangements of the various wigs for human and synthetic hair. Information provided by any offeror in response to this market survey shall not be disclosed if doing so would reveal the offeror?s confidential information and will be protected under FAR 3.104 or FAR Subpart 24.2. All offerors desiring to participate should submit their capability statements no later than July 21, 2005 @ 3:00pm.
