Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this Request for Proposals is: 528A6-11-05. This solicitation document incorporates provisions and clauses in effect through FAC 05-3. This solicitation is 100% set-aside for Small Business. The associated NAICS Number is: 334510 and the small business standard size is 500 employees. The VA Medical Center, Bath, New York, has a requirement for the following item(s): One (1) Parks Flo-Lab 2100 SX Arterial Vascular Doppler Ultrasound System which shall include: PACS Module (used to interface and send digital information); DIACOM and Sonova Vascular Diagnostic Software system; cuffs and accessories to include: (10 each) 12CM straight segmental cuffs, (2 each) extra-long 12CM straight cuffs, (2 each) 10CM straight segmental cuffs, (2 each) 17CM occluding tapered cuffs, (2 each) DC 1.9CM, (4 each) PC 2.5CM, (2 each) stiffeners, (2 each) extra bladders, Pentium 4 class computer w/Microsoft Windows XP software, 2.66 Hz, 512 RAM memory, 40 GB hard drive, CD-WR drive, 250 zip drive, broadcom nextreme Ethernet network adapter, deskjet color printer and 17? flat panel monitor. Complete unit shall be Parks Medical Electronics or equivalent. Delivery, which shall occur within 60 calendar days of contract award, shall be FOB DESTINATION to the VA Medical Center, Bath, New York. The Contractor is responsible for all freight charges. The provision at 52.212-1, Instruction to Offeror?Commercial Items, applies to this solicitation. The provision at 52.212-2, Evaluation?Commercial Items, applies to this solicitation. Paragraph (a) of FAR 52.212-2 is added to read: Award will be made on Best Value (ie: Technical Capabilities, Price). The provision at 52.212-3, Offeror Representations and Certification?Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with it?s proposal. A copy of the provision may be attained from http://www.arnet.gov/far/. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions?Commercial items. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive orders?Commercial Items, applies to this solicitation. The Offeror shall also provide its Commercial and Government Entity code, DUNS number and Tax Identification number. All contractual technical questions must be submitted in writing (email or fax). Telephone questions will not be accepted. As of October 1, 2003, all Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award.
