Description
THIS IS NOT A SOLICITATION ANNOUNCEMENT. This is a SOURCES SOUGHT SYNOPSIS only. The purpose of the Sources Sought Synopsis is to gain knowledge of potential qualified small business sources in Tobacco-Related Disparities for the Centers for Disease Control (CDC), NCCDPHP. The contractor shall provide professional services for highly specialized training on tobacco-related health disparities to be delivered to state department of health grantees. Because of the specialized topic area, the contractor needs to be knowledgeable in the National Tobacco Control Program (NTCP) and skilled in developing protocols for tobacco related activities within the disparied populations, implementing and developing training programs to train others to work with specific population groups in tobacco related disparities, and developing case studies as part of the qualitative evaluation for the NTCP. The contractor must have a thorough understanding of and experience with State Health department programs and in tobacco-related health disparities. Contractor shall have past experience in developing and implementing training for state health department personnel on the identification and limitation of tobacco related health disparities; further, contractor shall have previously developed case study evaluation protocols. Contractor shall have past experience as a state health department evaluation expert; especially in the area of tobacco-related health disparities program qualitative and quantitative evaluation. Contractor shall have a unique skill set that includes the following: ability to develop and implement training for state health professionals; ability to translate research on tobacco-related disparities into practice; ability to conduct large community assessments on health disparities; ability to work with state and national stakeholders on tobacco related health disparities; ability to provide training and technical assistance to a variety of groups and organizations on identifying health disparities; understanding of the CDC model for identifying and eliminating tobacco related health disparities; ability to develop and implement specialized training on addressing tobacco related health disparities for state health department grantees. This specialized, structured training will be delivered to grantees by three (3) two to three day trainings over the course of a year. Contractor will revise and/or develop highly specialized training materials and evaluation procedures, provide intensive, specific guidance to grantee on their case study qualitative evaluation process, and conduct an evaluation of the overall project. NAICS Code is 541990, $6.0 million. This will be 100 percent Small Business set-aside. All sources capable of providing these services are being sought. Capability statements need to demonstrate, through past experience or otherwise, an (1) understanding/familiarity with tobacco/health issues as they relate to the CDC (2) demonstrated capability to perform or manage the above services (3) financial capability to perform the services required. The Government shall not pay for any information submitted in response to this request. The projected period of performance will be a base year with two option years. The Government requests that interested offerors submit an electronic response of not more than two (2) one-sided pages, 8.5? x 11? paper, 12-pitch, Times New Roman font, with a minimum of one (1) inch margins all around.
