Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is FA8601-05-T-0137 and is issued as a request for quotation (RFQ). This RFQ is being conducted as a commercial item procurement in conjunction with Simplified Acquisition Procedures FAR Part 13. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-03. This solicitation and is issued as 100% SMALL BUSINESS SET ASIDE. Quotes are due by NOON (Eastern Standard Time), FRIDAY, 3 JUNE 2005, to the point of contact identified at the end of this notice. The associated standard industrial classification (SIC) code is 7999; the federal supply code (FSC) is U099; the North American Industry Classification System (NAICS) code is 611620; and the small business size standard is $6 million. The proposed purchase is for nonpersonal services from 15 Jun 05 through 30 Sep 05 for instruction of four fitness programs in accordance with the attached Statements of Work. The number of classes will not exceed the number provided in the Statements of Work. The Government may reduce the number of classes actually held after the contract is awarded. All sources providing quotes in response to this announcement are advised the Government may cancel some or all classes after contract is awarded without incurring additional cost to the Government beyond the number of classes actually held. Potential source shall separately quote all four instruction programs on the same quote. The unit price shall be the price to instruct one class: The four statements of work attached to this announcement are: 1) Instructor to provide standard Bootcamp Aerobics instruction 2) Instructor to provide standard aerobics and indoor cycling program for Mom and Baby Aerobics 3) Instructor to provide standard indoor cycling program for Indoor Cycling Program 4) Instructor to provide Fitness Equipment Orientation Program for fitness strength equipment Contract financing will NOT be provide for this acquisition. By submitting a quote, the quoter acknowledges the requirement that a potential awardee must be registered in the Central Contractor Registration (CCR) prior to award. The Government intends to award one firm fixed price (FFP) purchase order resulting from this solicitation to the responsible offeror whose quote is most advantageous to the Government and conforms to the requirements in this solicitation. The Government reserves the right not to make an award at all. Quotations may be in any format but must include requirements found in the attached Instructions to Offerors, including: company+s name, address and phone; point of contact+s name, phone, and email; quotation number and date; unit price (price per class); total price; identify any discounts; how long the quote is valid; and price. Quotations shall be accompanied by a completed Representations and Certifications (May 2004) and adhere to FAR 52.212-1, Instructions to Offerors ? Commercial (Jan 2004). Both of these documents are attached to this announcement.
