- NOTICE OF UNSOLICITED PROPOSAL. The Health Resources & Services Administration (HRSA) intends to negotiate, on a sole source basis, with the Arizona Department of Health Services, Phoenix, AZ 85007. This procurement will be processed in accordance with the Federal Acquisition Regulation (FAR) Part 13. The purpose of this acquisition is to conduct a pilot test in which dental hygienists function as case workers to track referral and treatment for comprehensive oral health care for Head Start Children. The contractor will share their data on the effectiveness of implementing the Head Start Triage System for Early Head Start Children age?s zero to three, their implementation manuals for the program, and their findings. Because the program is based on an unsolicited proposal, which the Government has determined to be an unique and innovative approach that was independently originated and developed by the offeror; without the Government?s supervision; benefits the Agency?s mission, and was not an advance proposal for a known agency requirement, the Arizona Department of Health Services is deemed the only responsible source capable of providing the required work and service under authority in 41 U.S.C. 253(d) (1) A, as set forth in FAR 6.302-1(a)(2) (i)(A). The Contractor has spent a substantial amount of effort in developing the pilot and will do this work with a large portion of the effort as an in-kind contribution. Any other source would have to duplicate a substantial variety of current and previous efforts to accomplish the same objectives. The anticipated period of performance is for twelve (12) months.
