Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes th e only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is W912EQ-05-Q-0024 and is being issued as a Request for Quotations (RFQ. This acquisition is 100% set-aside for small business. Women owned, Veteran owned, Total Service Disabled Veteran owned are all invited to submit quotations. The associated NAICS code is 423320 with 100 employees. All responsible sources may submit at quotation, which will be considered by the agency. Quotes may be submitted in hard copy (regular mail), electronically via Email or facsimile. Regardless of method of delivery, it is the offerors responsibility to ensure receipt of quotes by the time and date stated herein. Offerors shall submit quotes on line item s 0001 and line item 0002. The total price of line item 0001 and line 0002 will include all charges. Offerors must be actively registered with the Central Contractor Registration (CCR). The CCR website is www.ccr.gov. SECTION A  Statement of Work A-1. Scope  The work to be done under these specifications consists of furnishing all plant, labor, equipment and materials, and performing all operations in connection with the delivery of the culvert, flapgate and installation hardware in accordance wit h these specifications. SECTION B  DELIVERIES B-1. Deliveries  The items to be supplied under this purchase order shall be delivered to the job site beginning on or about 13 June 2005. Prior to shipment, the Contractor shall coordinate delivery with the following personnel. Primary - Richard Blake 901/544-0947 901/568-9174 (cell) Secondary - Richard Sullivan 901/544-0975 Rodger Funderburk 901/544-0728 Items 0001 and 0002 are to be delivered to a location approximately 6 miles east-northeast of Harrisburg, AR. The job site is on the west bank of Ditch 10, approximately 0.25 mi north of Highway 14. Line Item Number Description Estimated Quantity 0001 48 CMP 120 ft 0002 48 Flapgate 1 ea SECTION C - PAYMENT C-1. Payment. Payment will be made at the purchase order unit price for the quantity of acceptable material delivered. No payment shall be made for supplies replaced due to the Contractors fault or negligence. SECTION D  RENDITION OF INVOICES and VOUCHERS D-1. Submission of Invoices. Invoices shall be submitted in triplicate, typewritten or with pen and ink. Terms of payment must be stated, including cash discount, if any. D-2. Routing of Invoices and Vouchers. Invoices or vouchers shall be sent to the following address. USACE, Memphis CEMVM-OD-O, Room 510 (ATTN: L. Rowland) 167 N. Main St., B-202 Memphis, TN 38103-1894 SUPPLIES AND PRICES/COSTS 1. Pipe  1.1 The zinc-coated (galvanized) corrugated metal pipe culvert shall conform to the requirements of ASTM A760, Type 1 with bituminous coating and paved invert conforming to ASTM A849, Type A2. The manufacturers certified statement as to quality will be a ccepted in lieu of performing the prescribed tests. The pipe shall be fabricated from 0.079-inch (14 gauge) thick sheets. If helical corrugations are used, each length of helical pipe used shall have a minimum 12-inch length of annular corrugations at ea ch end. 1.2 The 48 diameter pipe shall be formed in 30 sections/lengths. Each section/length shall be fitted with two (2) lifting eyes. The lifting eyes shall be placed such that the weight of the pipe will be evenly balanced. 2 Coupling Bands  2.1 Requirement  Three (3) coupling bands suitable for use with the pipe referenced above. The coupling bands shall be 16 gauge (minimum) and a minimum of 12 wide with a 12 wide flat gasket. 3.1 Flapgate  Th e flapgate must be Fresno, Model 10c or equal (cast iron, spigot-back, low to medium head) or approved equal. All installation hardware shall be as recommended by the materials manufacturer. This solicitation document incorporates provisions and clauses of the FAR up to Federal Acquisition Circular 2005-03.
