Description
Solicitation for the possible replacement of Patient Hygiene Products for TRICARE Puget Sound and Alaska. These products include but are not limited to: no rinse cleaning alternatives: shampoo, bathing wipes, body bath, hair conditioner moisturizing lotion/creams and total body cleansers; variety of kits: shaving, dental hygiene, denture kits, hair care and personal bathing; dental products available in single packages: oral swabs, tooth brush, toothpaste, lip protectant and mouth wash. A variety of personal products available in single packages: moisturizers (lotions and creams), powder, liquid soap, shampoo (regular, dandruff and lice), hair conditioner, combs, razors, perineal care products, pre-shave, aftershave, shaving cream, shower cap and deo dorant/antiperspirant. Denture products available in single packages: denture cleansing tablets, denture brushes and denture cups. Patient belonging and garment bags. Single use total bath cleansing system available that can be used at room tempature or w armed. Single use total hair care system-shampoo and conditioning cap that can be heated. Those companies interested in a possible Regional Incentive Agreement (discounted DAPA pricing based on volume) with TRICARE Puget Sound and Alaska must respond to the following questions: 1.Does your company carry a full product line (no rinse, regular individually packaged and kits)? Describe your product line. 2. Is your company a manufacturer or a distributor? If your are a distributor what manufacturer(s) do you distribute? 3. Does your company have a distribution agreement with Cardinal? 4.Does your company currently possess or are in the process of obtaining a Distribution and Pricing Agreement (DAPA)? 5. Does your company have the ability to service the entire Region (WA and Alaska)? Provide your companys distribution network 6. Can your company provide customer support (1-800-numbers and product representatives for each account)? Provide numbers and names. 7. Does your company provide educational programs and clinical support? Describe material available and type of clinical support (brochures, video tapes, interactive access to web or other). Will product in-service and education be available upon re quest after product implementation throughout the length of a possible agreement? 8. Describe your companys return goods policy. 9. Describe your companys proposed implementation plan if you were to enter into an agreement with TRICARE Puget Sound and Alaska. 10. Does your company carry latex-free products? Provide a list of those products. 11. Are all products are clearly marked with contents? 12. Does your company provide products that are pH balanced, non-antibacterial, antibacterial and hypoallergenic? 13. Does your company provide products for the infant adult and pediatric population? 14. Are products available in gender neutral scent and fragrance free? 15. Length of time products have been on the market? 16. Does your company have products that can warmed for patient comfort? If yes provide a list of those products. 17. Have your products received Regulatory Agency Approval (FDA, EPA, OSHA, CDC other). Companies interested in participating in the evaluation and possible agreement with TRICARE Puget Sound and Alaska for Patient Hygiene products need to reply to this solicitation NO LATER THAN 6 June 2005. All responses will include company name, POC an d contact information (suggest more then one POC), your intent to participate in this standardization initiative and a reply to the above questions referred to by question number.
