Description

AMBULANCE/EMT SERVICES AT FORT POLK, LOUISIANA. Solicitation number W81K04-05-R-0024. Points of Contact are Ms. Christina Barr, Contract Specialist (210) 221-4127 and Mr. Gary Hankins, Contracting Officer (210) 221-3876. This requirement is for the Personal Services of Ambulance/EMT Services at BAYNE JONES ARMY COMMUNITY HOSPITAL, FORT POLK, LOUISIANA. The contractor shall be ALS/DLS certified as a paramedic or emergency medical technician (EMT). The contractor shall provide ambulance vehicles and supplies for each of the vehicles. The contractor shall have certified paramedics or emergency medical technicians who will provide Emergency Room support functions when not on ambulance runs. In addition, the base period of performance will be from 1 O ctober 2005 through 30 September 2009 with four 1-year options for the service area of BAYNE JONES ARMY COMMUNITY HOSPITAL, FORT POLK, LOUISIANA. This service shall be provided in accordance with Louisiana State Laws, Federal Laws, Army Regulations, and t he contract issued from this solicitation. The solicitation will be posted on FedBizOpps on or about 20 May 2005. This procurement is unrestricted for full and open competition. The NAICS code is 621910. The Small Business Size Standard for this requirem ent is $6,000,000.00.
