- The Department of Veterans Affairs is seeking a potential source to provide Community Based Outpatient Clinic (CBOC) Services to veterans as referred by the Department of Veterans Affairs, Veterans Affairs Medical Center (VAMC) Durham, North Carolina. The Durham VAMC participates in a veteran?s health administration program to provide outpatient clinical services to eligible veterans. CBOC services will be furnished to veterans in the community for whom such care is specifically authorized by Durham-VAMC. The services may include Primary Care, Mental Health, and Nurse Visits if required. Beneficiaries of this requirement shall be any eligible veteran residing primarily in Beafort, Edgecombe, Greene, Hyde, Martin, Pitt and Washington Counties but are not restricted to those counties. The NAICS code for this requirement is 621498 and Size Standard is $8.5 million. This is a sources sought notice only. No formal solicitation document exists at this time. Responses to this notice should include the following: name and address of company, Socio-Economic Factors, business size, if services are on a FSS, firms experience in providing Primary Care, Mental Health Services, and if required Nurses Visit Services and any other pertinent information which demonstrates firms ability to meet the above requirement. All responses shall be submitted in writing no later than close of business Friday, May 20, 2005, 12 Noon local time to the attention of: Ray C.
