- Description
- The Army Contracting Agency, Southern Region, Fort Polk Directorate of Contracting intends to issue a request for proposals to furnish all resources and management necessary to conduct a Welcoming Baby Program at Fort Polk. Contractor shall make ini tial contact with military parents of newborns at Bayne-Jones Army Community Hospital, Fort Polk, and/or home located in either Vernon or Beauregard Parishes, Louisiana either prior to or immediately following birth of baby, schedule home visits, educate p arents in child development, parent-child relationships/bonding, etc. An estimated 1224 visits are anticipated annually. NAICS code for this project is 624110 and the SIC code is 8322. Proposed contract will be a firm fixed price requirements contract w ith a base year and four one-year option periods. The Government will award a contract resulting from this request for proposal (RFP) to the responsible offeror whose offer conforming to the RFP will be most advantageous to the government. The following f actors will be used to evaluate: (1) Technical capability of the item offered to meet the Government requirement; (2) price and (3) past performance. The contractor shall be responsible for obtaining any necessary licenses, permits, and any professional t raining required. Proposals will be available for retrieval on or about 1 June 2005.
