Description

The Forest Service intends to make a single award for the provision of Optometry Services for the student population at the Pine Ridge Job Corps. The services will be performed on individuals at the Contractors place of business. The Contractor?s site shall be located within fifty highway miles of the Center. The Center will be responsible for transportation of students to and from the Contractor?s place of business. The contract type is a service contract with firm-fixed unit prices. The award will be made to the responsible quoter who submits the most advantageous quote to the Government based on price related factors only. The contract shall consist of a 12-month base period and four 12 month option periods for renewal. The following yearly estimated quantities are: 240 Professional eye examinations and 215 Prescriptions and eye glasses. The solicitation will be available on or about May 27, 2005 and will be distributed solely through the Federal Business Opportunities website at www.fedbizopps.gov. All future information concerning this acquisition, including solicitation amendments, will be distributed solely through this website. Interested parties are responsible for monitoring this site to ensure you have the most up-to-date information about this acquisition. Hard copies of the solicitation will not be available. The website provides all downloading instructions to obtain a copy of the solicitation. The anticipated date for receipt of quotes is on or about June 27, 2005. The solicitation is unrestricted and all sources may submit a quotation. All responsible sources may submit a quote, which will be considered.
