- Customs and Border Protection, Grand Forks Sector (Grand Forks, North Dakota) is requesting a firm-fixed price quotation for physical exams for 23 (twenty-three) male and 3 (three) female employees over the age of 40 (forty). This is a combined synopsis/solicitation for commercial items prepared in accordance with format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. One contract is anticipated to be awarded as a result of this solicitation. The term of this contract is the one year from date of the award. At least one physical exam shall be conducted on or before September 30, 2005. The contractor shall provide all personnel, materials, services, facilities and any other items and services necessary to conduct Physical Examinations for employees over the age of 40. The contractor shall provide Physical Examinations during the hours of 8:00am through 4:00pm, Monday through Friday, excluding Federal Holidays. The contractor shall provide the workforce possessing the skills, knowledge, and training to satisfactorily perform the services required by the contract. The contractor?s workforce shall be board eligible or certified by their appropriate occupational health or medicine discipline. The contractor?s facilities shall be located in or around a 30-mile radius of Grand Forks, North Dakota. The performance of the services will occur at the contractor?s facility. The Contractor awarded the contract must obtain a Data Universal Number (DUNS), and register in the Central Contract Registration System (CCR). The provisions of FAR 52.212-1, Instructions to Offerors-Commercial; FAR 52.212-2, Evaluation-Commercial Items; FAR 52.212-4, Contract Terms and Conditions-Commercial Items; and, FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, apply to this acquisition. FAR 52.222-35, Affirmative Action for Disabled Veterans and Veterans of the Vietnam Era. FAR 52.222-36, Affirmative Action for Handicapped Workers. FAR 52.222-37, Employment Reports on Disabled Veterans and Veterans of the Vietnam Era. FAR 52.222-3, Buy American Act-Supplies. The OFFEROR shall include the firm's DUNS (Data Universal Numbering System) number as part of their offer. The Government will award a contract resulting from this solicitation to the responsible offeror with the lowest price whose quote conforms to the RFQ.
