Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The NAICS Code for this synopsis/solicitation is 611620, Size Standard $6M. Solicitation/Purchase Request number FOX51N5182100 is issued as a Request for Quote (RFQ). This document incorporates provisions and clauses that are in effect through Federal Acquisition Circular 2005-03, effective 11 April 2005. The Government intends to award one Contract Line Item Number (CLIN 0001) for the work described in the below Statement of Work (SOW); QUANTITY: 1 YR. DESIRED PERIOD OF PERFORMANCE: 1 May 2005 - 30 April 2006 FOB: DESTINATION. Inspection and Acceptance: DESTINATION. The following provisions and clauses apply to this procurement: FAR 52.212-1, Instructions to Offerors-Commercial Items; In accordance with the FAR 13.106-1(a)(2) offerors are notified that award will be made on the basis of price and past performance. Offerors are required to submit at least three references for past performance. Price and past performance are of equal importance. Offeror will be evaluated in accordance with FAR 13.106-2. GENERAL PROVISIONS LIST OF FAR CLAUSES INCORPORATED BY REFERENCE The provisions of the following clauses set forth in the Federal Acquisition Regulation (FAR) or DoD FAR Supplement are hereby incorporated into this order or contract by reference with the same force and effect as though herein set forth in full. As used in the following clauses, the term "Government" is deleted and the abbreviation "NAFI" is substituted in lieu thereof. The date of each clause shall be the current date set forth in FAR or DoD FAR Supplement on the issuance date of this order or contract. Clauses made inapplicable by the reference or by the kind of order, contract (e.g., orders or contract for services instead of supplies) or dollar amount are self-deleting. The complete text of any clause incorporated in this order or contract by reference may be obtained from the contracting officer. CLAUSE NO. Be advised that all interested parties must be registered in the Central Contractor Registration (CCR) Database in order to receive an award. This is a nonappropriated fund purchase and it does not obligate appropriated funds of the United States Government. Nonappropriated funds are generated by the military community through the sale of goods and services and the collection of fees and charges for participation in military community programs. This purchase does not involve federal tax dollars. Statement of Work Personal Trainer Schedule of Services and Compensation 1. SERVICES TO BE PROVIDED AND PERIOD OF COVERAGE: The contractor will conduct Personal Training Instruction at the Fitness Center at Hanscom AFB MA for a period of one year, 1 May 2005 through 30 Apr 2006, or as long as participation warrants. Instruction will last a maximum of 120 minutes. All levels of instruction will be taught. 2. PARTICIPATION: Participation in these training sessions is restricted to dependents of military personnel, retired personnel and Department of Defense (DOD) employees and eligible patrons of Services Programs here at Hanscom AFB. 3. CONTROL OF ATTENDANCE AND FEES: The Fitness Center personnel will collect fees from participants. All fees are due to the Fitness Center prior to instruction. The instructor will turn in an attendance sheet for each day training sessions are held, listing attendance for each class. 4. CANCELLING AND RESCHEDULING TRAINING SESSIONS: The instructor is responsible for notifying the Fitness Center personnel if instructor is not able to hold training session. If training sessions are not held because of bad weather, or duty commitment, the Instructor will be responsible to notify the patrons of the change and when the next class will be held as well as time. If instructor cancels training sessions, the instructor is responsible for rescheduling a new time with students. If the patron cannot make the scheduled new time, instructor forfeits all fees due and student is not responsible for the training session payment. 5. CERTIFICATION AND INSURANCE: The contractor shall have National Certification through a qualified Fitness Association. (AFAA, ACE, etc.) and must submit proof of the certification. The contractor must have liability insurance and submit proof of coverage. The contractor shall have extensive knowledge of and practice in physical training. 6. PAYMENTS: COST/PRICE: The Government shall charge the customer a cost for massage therapist services based on the price paid by the government to the contractor. Charges should be on an hourly basis. The price charged by the contractor will have a 10% mark up that will go to the Government. Contractor shall be paid the amount agreed to by both parties upon award of contract. Payment will be made by the 10th working day of the following month by electronic payment, provided the proper invoice has been received from the instructor by the 3rd working day of the following month. 7.
