Description

RFQ 786-99-05 - Furnish and Install Concrete Grave Liners and Casket Transports @ Ohio Western Reserve National Cemetery, Rittman, OH Contractor shall provide all necessary labor, supervision, equipment, and materials to furnish, deliver (F.O.B. Destination within consignee premises) and install concrete grave liners to include casket transport at Ohio Western Reserve National Cemetery, 10175 Rawiga Road, Rittman, OH 44270. Contract covers base period June 1, 2005, or date of contract award through September 30, 2005, with up to four 1-year options to extend the term of the contract through September 30, 2009. This acquisition is a total (100%) set-aside for small business concerns. North American Industrial Classification System (NAICS) Code 327390 is applicable. Request for a hard copy of the solicitation must be in writing to Mrs. Michelle Cruz, Purchasing Agent, and accompanied by a check or money order in the amount of Twenty-Five Dollars ($25.00) (non-refundable), payable to ?U.S. Department of Veterans Affairs.? Please reference Solicitation Number RFQ 786-99-05 on remittances. No telephone requests will be accepted. Hard copy solicitations will be issued via the ordinary mails on or about April 22, 2005. The solicitation may also be obtained FREE OF CHARGE, from VA?s Business Opportunities System via the Internet at the following URL address on or about April 22, 2005: http://www.bos.oamm.va.gov/solicitation?number=786-99-05. Any amendments to the solicitation will also be available at the URL address. Offerors are advised that they are responsible for obtaining amendments. See Note 1.
