COMBINED SYNOPSIS/SOLICITATION36th CONTRACTING SQUADRON/LGCBANDERSEN AFB, GUAM This is a combined synopsis/solicitation for commercial items prepared per the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Requests for quotations are being requested. No other written solicitation will be issued. GENERAL INFORMATION Solicitation Number: F1C2MU5081A100 Request for Quotation (RFQ) 1 Request for Proposal (RFP) 0 Invitation for Bid (IFB) 0 This solicitation is issued as a request for quotations (RFQ). This RFQ incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 97-27. This acquisition is set-aside for small businesses only. Large businesses may not submit an offer. North American Industry Classification Code (NAICS): 484220 Size Standard: $21.5 M LINE ITEM DETAIL Line Item 0001: All service are to be performed in strict accordance with the attached Statement of Work.The contractor shall provide all labor and transportation necessary to obtain and deliver 20 - 20 foot chassis and to lease the chassis to the United States Air Force at Andersen Air Force base, Guam. The lease shall be effective from 01 May ? 01 August 2005. The lease period for a particular chassis will start when that chassis is delivered to and accepted by the 36 Maintenance squadron at Andersen AFB. The chassis shall be in new working order and capable of passing an inspection in accordance with CFR 49 Federal Law for shipping hazardous materials. The lease will include delivery, pick-up, and all maintenance with the exception of tire changes and maintenance/repairs resulting from the misuse or abuse of the chassis. SPECIAL NOTICE The Contracting Officer is the only individual authorized to direct changes, interpret clauses, or otherwise modify the terms or conditions of this contract. Deviations from requirements specified in this contract, without written authorization from the Contracting Officer, will be at the contractor's risk and may not be compensated for such deviations. PERIODIC PROGRESS MEETINGS (a) The Contracting Officer, QA and other government personnel will meet as required with the contractor to review the contractor's performance. At these meetings, the Contracting Officer will apprise the contractor of how the Government views the contractor's performance and the contractor will apprise the Government of problems, if any, being experienced. Appropriate action will be taken to resolve outstanding issues. (b) The written minutes of these meetings will be signed by the Contracting Officer, contractor's representative and functional personnel as appropriate, then distributed to the functional area and the contractor. The contractor will provide a letter to the Contractor Officer stating any areas of disagreement and rationale for this disagreement for action and/or filing with minutes. ENVIRONMENTAL COMPLIANCE (a) Hazardous Material Identification. The latest version of Federal Standard No. 313 (Material Safety Data Sheet, Preparation and submission of) includes criteria for identification of hazardous materials. Hazardous material data (Material Safety Data sheets (MSDS)) is required- 1. As specified in the latest version of Federal Standard No. 313 (including revisions adopted during the term of the contract; 2. For any other material designated by the Government technical representative as potentially hazardous and requiring safety controls. 3. MSDSs must be submitted by the apparently successful offeror prior to contract award, unless the offeror certifies that the supplies are not hazardous. (b) HAZMART. The Government will inventory all chemicals that the contractor brings on to Andersen AFB or any property under the control of Andersen AFB. Any products that meet the criteria for "Hazardous Waste," must be bar coded and tracked until permanently removed from Government property. The inventory will be performed at HAZMART facility, prior to commencement of work. Criteria for identifying hazardous waste, is contained in Subpart C of 40 CFR, Part 261. (c) Environmental Impact. All waste materials generated by the contractor, or any work under the contract performed on a government installation, shall be handled, transported, stored, and disposed of by the contractor and by his subcontractors at any time in accordance with all applicable Federal, state, or local laws, ordinances, regulations, court orders, or other types of rules, or rulings having the effect of law, including, but not limited to, Executive Order 12088, 13 October 1978; the Federal Water Pollution Control Act, as amended (33 USC Sec 1251 ET SEQ); the Clean Air Act, as amended (42 USC SEC 1857 ET SEQ); the Endangered Species Act, as amended (16 USC Sec 1531, ET SEQ); the Toxic Substance control Act, as amended (15 USC Sec 2601 ET SEQ); the National Historic Preservation Act, as amended (16 USC Sec 470, ET SEQ); and the Archaeological and Historic Preservation Act, as amended (16 USC Sec 460, ET SEQ). POLLUTION PREVENTION (P2) (a) In accordance with Air Force Pollution Prevention Strategy, Andersen AFB intends to effectively promote pollution prevention by minimizing or eliminating the use of hazardous materials and the release of pollution into the environment. As part of this initiative, contractors shall comply with all Federal, State, and Local environmental laws, regulations, and guidelines. In addition, the following special requirements shall apply: (1) The provision AFFAR 5352.223-9000. (2) The provision Environmental Compliance. (b) Contractors are encouraged to participate in the following voluntary programs: (1) Affirmative Procurement - purchasing of supplies and services using recycled products (buying green products). (2) Recycling of waste products to the maximum extent possible. (3) Energy Conservation. GUAM GROSS RECEIPTS TAX Contract unit prices proposed for all CLINs shall include Guam Gross Receipts Tax (GRT). Invoices for these CLINs must identify the amount of GRT included in the total payment requested, as noted below: The total payment requested by this invoice includes $ of Guam GRT. Offerors are advised that all contract line items are subject to Guam's Gross Receipts Tax (GRT). PRIVACY ACT STATEMENT The contractor understands that the property under this contract may contain records previously maintained as a system of records subject to the Privacy Act, 5 U.S.C. 522(A).
