- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice for the South Texas Veterans Health Care System. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. This procurement is being issued as a Request for Quotations (RFQ), and the reference number is 671-28Q-05. This solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular 2005-2. The NAICS code is 423910 and the size standard is 100 employees. Prospective vendors are required to submit written proposal to include transportation, labor, equipment, tools and insurance for the period of performance to Veteran participants of the 25th National Veterans Wheelchair Games (NVWG) being held in Minneapolis, Minnesota during the period of June 26-30, 2005. Contractor will provide a climbing wall that is adaptable to accommodate various degrees of disabilities. Contractor must have past experience dealing with disabilities and accomodating the special need requirements to provide a safe environment for participants. Contractor will be responsible for providing the adapted climbing wall, all transportation, set-up, break-down, equipment, instruction and staff operation of the wall. Additionally contractor must provide insurance coverage for disabled participants. The Government anticipates awarding a single firm-fixed price contract to the contractor who can meet the government's requirements in it's entirety. All quotes shall include pricing for F.O.B. destination to the NVWG site located at Minneapolis, Minnesota. The following FAR clauses and provisions are applicable to this solicitation: ALL OFFERORS ARE TO INCLUDE A COMPLETED COPY OF THE PROVISIONS AT 52.212-3, OFFEROR REPRESENTATIONS AND CERTIFICATIONS COMMERCIAL ITEMS WITH OFFER. The following FAR clauses and provisions are applicable to this solicitation: 52.212-1, Instructions to Offerors-Commercial Items, 52.212-4, Contract Terms and Conditions--Commercial Items; 52.204-7 Central Contractor Registration; 52.247-34 F.o.b. - Destination, 52.217-9 Option to Extend the Term of the Contract(30 days/60/4 years); 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercials Items; The following FAR clauses identified at paragraph b of 52.212-5 are considered checked and are applicable to this acquisition: (5),(11-20),(24ii),(32) section (c) (1),(2),(3), 52.232-19 Availability of Funds for Next Fiscal Year, 52.219-1 Small Business Program Representations, VAAR 852.219-70 Veteran-owned Small Business, VAAR 852.270-4 Commercial Advertising. Wage Determination Number 1994-2287(Rev 27) applies.
