- 17. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are not being requested for the solicitation number NOI5062. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-02. This acquisition will be processed under Simplified Acquisition Procedures (SAP) and is a small business concern. The associated North American Industry Classification System (NAICS) Code 811310, which has a business size standard of 6.0. The National Institute of Allergy and Infectious Diseases intends to award a sole source procurement with Carl Zeiss Microimaging for a one year maintenance agreement service(s) on two Microscopes: 1) Radiance 2100 Multiphoton/Confocal Visible Imaging System, 2) Radiance 2100 Multiphoton/Confocal Imaging Systems located at NIH in Bethesda, Maryland. The period of performance will be for one year. The proposed contract is for supplies/services for which the Government intends to solicit and negotiate w/only one source under authority of FAR 6.302. Only one responsible source and no other supplies or services will satisfy agency requirements. ?See Note 22?. The following FAR provisions and clauses apply to this acquisition: FAR 52.212-1 Instructions to Offerors Commercial Items; FAR 52.212-3 Offeror Representations and Certifications Commercial Items; FAR 52.212-4 Contractor Terms and Conditions Commercial Items; FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items. A completed copy of provisions at FAR 52.212-3 Offerors Representations and Certifications Commercial Items must be included. Copies of the aforementioned clauses are available upon request by telephone to Ms. Tonia Alexander at (301) 496-2607. Offers must be submitted no later than 5:00 PM Eastern Daylight Time (EDT) April 18, 2005 to Tonia Alexander.
