- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Defense Information Systems Agency (DISA) is isssuing a solicitation for the purchase of 114 pre-terminated measured fiber runs and 228 12-port fiber patch panel MPT cassettes for ELTC39 Lab modernization. The product list and MTP specs are attached. Solicitation HC1047-05-T-4021 is being issued as a Request for Quotation (RFQ), which incorporates provisions and clauses for commercial items in effect through Federal Acquisition Circular 2001-14. This acquisition is reserved for small businesses. The NAICS code for this acquisition is 334119. The small business size standard is 1000 employees. The delivery date will be 4-6 weeks after receipt of order. The place of delivery will be 701 N. Courthouse Road, Building 15, Arlington, VA 22202, ATTN: Kent Smith. The acceptance and FOB point is destination.The following provisions in their latest editions apply to the solicitation: FAR 52.211-6, Brand Name or Equal; FAR 52.212-1, Instructions to Offerors ? Commercial Items; 52.212-2, Evaluation-Commercial Items; 52.212-3, Offeror Representations and Certifications-Commercial Items; 52.212-4, Contract Terms and Conditions-Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items. All quotes will be evaluated on price, delivery and conforming with specifications. Please submit with your quote a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items. The Government will award a contract to the responsible offeror whose offer is conforming to the requirements as stated in this synopsis and will be most advantageous to the Government. The FAR clauses referenced above may be viewed at http://farsite.hill.af.mil. Contractors submitting quotes must be enrolled in the Central Contractor Registration (CCR) program (http://www.ccr.gov/).
