Description
The following (2) changes modify Solicitation No. TFMSHQ05Q00010, (1) The Solicitation is not set-a-side for small business it is a full and open Solicitation/Requirement, (2) Performance Period for base year only is changed from Base Year April 1, 2005 through September 30, 2005 to read May 1, 2005 through September 30, 2005, all other option years and terms/conditions will remain the same as in solicitation and unchanged. This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Solicitation No. FHQ05Q00010 is being issued as a Request for Quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-27. The National American Industry Classification System code is 621999 with a corresponding size standard of $8.5M. The size status for this procurement shall be identified in the quote. The Department of the Treasury, Financial Management Services (FMS), 401 14th Street, SW, Washington, DC 20227, is seeking sources of a commercial, Healthcare Services and the necessary personnel, management supervision, supplies, and medical equipment to conduct a comprehensive program of occupational health and wellness which includes, but not limited to on-the-job injuries or illnesses, various screenings, examinations, health profiles and counseling, as described in the following: (1) Hours of Operation - Except federal government holidays or building closures, each of the employee health and wellness centers shall be open Monday through Friday at two different sites, (la) Liberty Center Building located at 401 14`h street, SW, Washington, DC 20227, time 7:45am - 4:15 pm and (lb) PGMD-II, 3700 East West Highway, Hyattsville, MD 20782, 7:30am to 4:00pm. The health and wellness centers shall be closed at lunchtime with the exception of the days when there are two nurses working at the PGMC-II facility. In that instance, the nurses shall rotate lunch hours so that the health unit is open for service throughout the day. In the event the government agency building must close for the entire day or delay opening due to weather conditions, an announcement will be broadcast on local TV and radio stations. (2) Standards of Performance - In as much as the contractor will be interacting with agency personnel on a daily basis, the staff shall conduct themselves in a professional manner consistent with the expectations of the medical profession. (3) Description of Services - The contractor shall provide a wide range of non emergency and emergency occupational health care to include, but not limited to, direct medical treatment, physical examinations, prevention, and wellness services such as, but not limited to, counseling in nutrition, and stress management. (4) Maintenance of records - The contractor shall maintain secure, accurate, and organized records. In addition, the contractor shall maintain patient confidentiality. The contractor shall maintain these records in accordance with FMS records management guidelines. (5) Non-Emergency Services - Non-emergency services can typically be described as those which require the physician to examine and/or counsel employees, provide consultative services to agencies, administer medication, assist employees in identifying an outside practitioner when necessary and maintaining confidential and complete medical records of the services provided. For purposes of this contract, non-emergency services typically include: (5a) Examination of employees seeking general medical attention, including obtaining and evaluation medical history: (5b) As an aid in preventing and controlling health risks, the contractor shall be required to appraise and report work environment health hazards brought to the physician's attention through any illness or injury presented to the physician which may have resulted from a hazardous or unhealthy condition in the building. In this situation the physician's expertise and advice will play an important part in the formation of FMS developing an action plan to deal with newly identified work environment hazards. (5c) Providing health education counseling to encourage employees to maintain personal health, such as, but not limited to, non-smoking programs and stress management; (5d) Providing counseling to employees on issues such as, but not limited to, alcoholism and drug abuse; (5e) Providing disease screening examinations which are determined to be necessary and/or in the best interest of the employees. Examples of such examinations include, but not limited to, blood pressure screening for hypertension and skin test; (5f) Conducting simple vision screening upon an employee's allegation of a vision problem; (5g) Administering immunizations on a voluntary basis, as part of agency sponsored program, such as but not limited to, flu shots. (5h) Administration and/or monitoring of treatments and medications furnished by the employee and prescribed in writing by the employee's personal physician, e.g., administering allergy shots. The administration and or monitoring of treatments or medications shall be limited to those areas considered to be within the capabilities of the health and wellness center facilities and equipment, and consistent with the training, knowledge and experience of the physician providing services under this contract. (5i) Clarifying or interpreting medical terminology appearing in documentation prepared by an employee's private physician and presented to the agency regarding the employee's physical and/or mental fitness conditions which are allegedly impacting the employee's performance and/or issues of medical fitness with respect tot reasonable accommodations of an allegedly handicapping condition. The contract physician may be requested to contact an employee's private physician for the purpose of obtaining clarification of obtaining clarification of the documentation provided or to obtain additional information. (6) Emergency Services - The contractor shall provide emergency medical treatment, including diagnosis and initial treatment of illness or injury that becomes necessary during work hours, whether or not such injury/illness was sustained by a Government or contractor employee while in the performance of duty. In emergency cases beyond the treatment capabilities of the health and wellness center it is expected that the health care professional will notify 911 emergency services and transport a patient to a hospital emergency room or other medical facility when necessary. Under no circumstance will the employee be allowed to be taken to the hospital by another government employee or to drive him/herself. (7) Occupational Treatment - Employees who have an occupational injury or illness will receive, if they elect, medical treatment within the capability of the facility. When the patient requires treatment beyond the capabilities available, he or she shall be transported to an emergency room for treatment if necessary, or referred to an appropriate physician or a physician of the patient's choice. The contractor shall inform employees reporting to the Health and Wellness Center with an occupational injury or illness to notify the Financial Management Services' Occupational Workers Compensation Programs representative of their situation. The OWCP representative shall be responsible for providing the employee the necessary forms. (8) Non-Occupational Treatment - Employees who experience a non-occupational illness during working hours will receive, if they elect, primary diagnosis and treatment with subsequent referral to a physician. Initial treatment will be given for medical conditions for which the individual could not reasonably be expected to seek the attention of a personal physician, or to enable the individual to complete the current work shift before consulting a personal physician. (8a) Contract medical personnel will be expected to maintain an emergency medical kit including an Automatic External Defibrillator (AED) in constant readiness for response to emergencies. (9) Health Counseling - The contract physician or nurses shall provide health counseling to individual employees as necessary from sources such as, but not limited to, physical examinations, referrals from other health care providers, such as but not limited to, personal physicians, and observations from supervisors or management personnel. (9a) Employees with alcohol or drug abuse, emotional or behavioral disorders, or other personal problems that may affect job conduct or performance shall be counseled and referred to the Employee Assistance Program counselor. When necessary, the employee shall be referred to community or other resources in such manner as will permit appropriate and skillful management of employee problems. (10) Health and Wellness Education - The contract physician or nurses shall present or arrange for a minimum of six (6) health education programs or seminars each contract year, and make available health related pamphlets and literature. (11) Fitness for Duties Examinations - Fitness for duty examinations shall be performed as requested by FMS Human Resources staff as follows. (1 la) On and emergency basis for employees who are thought to have acute health problems, which might create a hazard to themselves or others, such as being under the influence of alcohol or other drugs. (11 b) For employees whose work performance is thought to be adversely impaired by health problems. (11c) For employees returning to work following major illness or major surgery or following rehabilitation. Fitness for duty examinations often require several group conferences with supervisors and the Office of Human Resource staff, and all require written reports to be sent to the Personnel Officer. The health and wellness center staff physicians shall provide clear and definitive medical or psychiatric judgments pertaining to the medical problem and/or work restriction as it relates to the specific job of the employee. The staff physicians shall work closely with the FMS Office of Personnel Management in establishing job requirements on a case-by-case basis. (12) Physical Examinations (Basic) - The contractor shall conduct comprehensive physical examinations, and fitness for duty examinations. The physical examinations shall be on a voluntary basis, and include, but are not limited to the following. (12a) General medial history, including family background and genetic factors. (12b) Occupational history, including environmental factors and work place hazard. (12c) General physical examination. (12d) Blood chemistry such as SMA-32 or better which would include cholesterol and triglycerides and a urinalysis. (12e) Complete blood count including RBC, WBC, differential, hematocrit, hemoglobin, MCH, MCV, and MCHC. (120 Electrocardiogram for patients over 40 years of age or as indicated by contract physician. (12g) Tuberculin skin test. (12h) Vision testing to include standard test for color blindness. (13) Employee Assistance Program (EAP) - After initial assessment, the contract nurses or health care physicians shall refer employees to the EAP as deemed necessary. (14) Personnel Qualifications - The Contractor shall provide required services, at a minimum, fully qualified personnel such as, but not limited to Staff Physicians, Chief, Charge, and Staff Nurses. The Contractor shall be responsible for certifying that they meet all personnel qualifications specified. (14a) All personnel's necessary credentials, such as resumes, licenses, and certification associated with his or her position shall be provided to the Contracting Officer (CO). In the event of an extended absence of a staff member listed below, the Contracting Officer's Technical Representative (COTR) shall receive written notification of the personnel's absence. (15) Medical Director - The contract Medical Director shall be required to be physically located on site, at the health and Wellness Centers that employ a Medical Director. The contract Medical Director shall perform management oversight over the contract requirements and provide medical direction to the staff. The Medical Director shall use a variety of marketing strategies to encourage employees to use the Health and Wellness Center. (15a) The Medical Director shall also provide, as requested feedback to management on conditions in the FMS Liberty Center Building, Washington, DC and FMS PGII Building, Hyattsville, MD that require corrective actions such as, but not limited to, environmental issues effecting employee safety, health and wellness. To this end, the contract Medical Director shall develop and conduct a structured medical surveillance program, using new and existing information obtained from reviewing employee physical examinations as a baseline. The medical surveillance plan shall meet the approval of the COTR prior to implementation. (15b) Education - The medical Director shall be a graduate of an accredited medical school. (15c) Certified by the American Board of Internal Medicine with expertise in primary care, internal medicine, or family practice (except pediatrics or obstetrics/gynecology); and licensed to practice in the Washington, DC and Hyattsville, MD metropolitan areas. (15d) Certified to administer cardiopulmonary resuscitation (CPR). (16) Experience - The Medical Director shall have professional experience (one year may include internship) that includes management of an occupational medical program and thorough knowledge of occupational medicine. (16a) Experience in providing addiction and dependency counseling and referring employees to appropriate professional assistance. (16b) Experience in identifying a broad range of behavioral problems; and detailed knowledge of the Washington, DC and Hyattsville, MD metropolitan community treatment resources for patient referrals. (17) Staff Physician - The contract Staff Physician shall be required to provide medical treatment to sick or injured persons at each specified facility. Since the physicians may have outside practices he or she may not make referral to his/her own practice or affiliates or other wise act as any employees' private physician. Furthermore, this policy may not be circumvented by implying that some other staff member made the referral or that the individual referred himself/herself to the physician. (17a) Education - The Staff Physician shall be a graduate of an accredited medical school, certified by the American Board of Internal Medicine with expertise in primary care, internal medicine, or family practice (except pediatrics or obstetrics/gynecology); and licensed to practice in the Washington, DC and Hyattsville, MD metropolitan areas. (17b) Certified to administer cardiopulmonary resuscitation (CPR). (18) Experience - The Staff Physician shall have professional experience (one year may include internship) in occupational medicine techniques and philosophy; experience in counseling troubled employees. (18) Chief Nurse - The contract Chief Nurse shall be required to assist the physician with treatment, assess patient health problems and needs; maintain medical records; and assist patients in complying with a prescribed medical regime. The Chief Nurse shall oversee the staff nurse's performance and assist with any and all medical treatments necessary to operate the specified facility. (19) Education - The Chief Nurse shall be a graduate of Baccalaureate degree nursing program. (19a) Licensed to practice in the Washington, DC and Hyattsville, MD metropolitan areas. (19b) Certified to administer the American Heart Association modular cardiopulmonary resuscitation (CPR). (20) Experience - The Chief Nurse shall have experience in occupational health nursing with supervisory experience. (20a) Experience in an industrial medical program, or in ambulatory or emergency care. (20b) Experience in counseling troubled employees; and experience in conducting special medical programs or health education. (21) Staff Nurse - The contract Staff Nurse shall be required to assess patients health problems and needs, and provide services such as, but not limited to, administering immunizations, inoculations, allergy treatments, medications, and maintain medical records. (22) Education - The Staff Nurse shall be a graduate of Baccalaureate degree nursing program. (22a) Licensed to practice in the Washington, DC metropolitan area (DC and MD or in the areas the services will be performed. (22b) Certified to administer the American Heart Association modular cardiopulmonary resuscitation (CPR). (23) Experience - The Staff Nurse shall have professional nursing experience in an industrial medical program, or in ambulatory or emergency care, experience in counseling troubled employees and experience in conducting special medical programs or health education. (24) Key Personnel - Key personnel are understood to be those individuals for whom resumes were submitted as part of the proposal to this solicitation. (25) Permits, License, and Insurance During performance of an contract awarded in response to this Solicitation, the Contractor shall maintain liability insurance coverage, from a carrier acceptable to, The Financial Management Service for all personal injuries, property damage and/or wrongful death arising from performance of the Contract. Prior to beginning performance, the Contractor shall provide the CO with certificates evidencing as insurance coverage. During performance of any contract awarded in response to this Solicitation, the Contractor shall indemnify and hold harmless the federal government all claims for personal injury, property damage, and/or wrongful death arising from performance of the contract. All quotes must incorporate a transition time period that will allow for adequate set up time. Which should be not less than 15 days and not more than 30 days. Performance Period for these services are Base Year beginning May 1, 2005 through September 30, 2005 and (4) one year Options, Option Year (1) October 1, 2005 through September 30, 2006, Option Year (2) October 1, 2006 though September 30, 2007, Option Year (3) October 1, 2007 though September 30, 2008, Option Year (4) October 1, 2008 through September 30, 2009. All qualified business sources are encouraged to submit a proposal for which the Agency shall consider. Solicitation provisions at FAR 52.212-1, Instructions to Offers-Commercial Item (Oct 2000) are hereby incorporated by reference. The Government will evaluate quotes in response to the synopsis/solicitation and award on the basis of Technical (Staffing, qualifications/experience, Past Performance and Price. Also, not excluding conformance to the stated Statement of Work (SOW) with company insurance credentials are major factors. It is critical that quotes provide adequate detail to allow evaluation (see FAR 52.212-1(g). Include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items (Oct 2000), with the quote. Clause 52.212-4, Contract Terms and Conditions-Commercial Items (May 1999), is hereby incorporated by reference. Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (Aug 2000), is hereby incorporated by reference. The following clauses apply to this solicitation and the resultant contract (b): 52.203-6, 52.222-21, 52.222-26, 52,222-35, 52.222-36,52.222-37, 52.225-1, 52.225-3, 52.232-34, Addendum to 52.212-5, Clauses 52.217-8 Option to Extend Services (Nov 1999) the Contracting Officer may exercise the option by written notice to the Contractor within (60 days), Option to Extend the Term of the Contract (Mar 2000) PERSONNEL SECURITY REQUIREMENTS FOR SENSITIVE DUTIES. The FMS Security Office has designated the contractor services as described in the statement of work for this contract at the (To be determined) risk level and will require a background investigation prior to beginning work on this contract.
