Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The proposed contract is set-aside 100% for small business concerns. The North American Industry Classification System (NAICS) code is 611710 size standard is $5.0 This requirement is for a Base Year + 4 Option Years. STATEMENT OF WORK. Contractor will work based on the need, to provide classes for the installation. Contractor may work in the range of 20 to 40 hours a week based on the need to deliver the below listed services. Services will be provided Monday through Friday from 0800-1700. Some evening and week-end classes may be required. a. Provide classes in the education and prevention of spouse and child abuse to each military unit annually. b. Contact each unit trainer directly to schedule the annual spouse and child abuse mandatory awareness training requirement IAW AR 608-18. c. Maintain statistical records on classes conducted. Provide statistical input weekly for the SITREP and monthly for the FAP portion of the ACS Management Report to the COR/Family Advocacy Program Manager. d. Assess post-wide Family Advocacy troop education and training program needs in order to make improvements in service delivery. e. Provide assistance in the facilitation of other prevention programs such as Prevention and Relationship Enhancement Program (PREP), A Couples Journey, Special FAP Events, Family Readiness Group and/or Rear Detachment presentations, Fathering c lasses, Family Life Skills classes, parenting classes (e.g. Cooperative Parenting & Divorce, Nurturing PEACE), Deployment/Reunion classes, or other programs as needed by the Family Advocacy Program. C.4. PERFORMANCE OF WORK. a. The non-personal services herein called for are to be performed at various locations on Fort Carson and the surrounding community. b. Performance will normally occur during Army Community Services established hours of operation, 0730-1615, Monday-Friday. C.5. MINIMUM QUALIFICATIONS. a. Have at least two years experience teaching or facilitating educational classes to military personnel or families. b. Have demonstrated knowledge of spouse and child abuse dynamics. c. Have two years experience providing social services to soldiers and families in a military community or setting. d. Have a Bachelors Degree from an accredited college or university in a social/human service field. C.6. ADMINISTRATION a. All information, training materials, publicity, and records pertaining to the contracted service will be the property of the U.S. Army. b. The United States Government may provide to the Family Advocacy Trainer training in Family Advocacy Programs and concepts. c. The United States Government will provide space in which to conduct troop education and other prevention classes, office supplies, commercial class A telephone access, office equipment to include desk, chair, use of computer, training equipment such as projectors and VCR, graphic support to prepare training aids and publicity, and copy reproduction services. END OF SECTION C CONTRACT DELIVERABLES 1. Contractor will ensure the following Family Advocacy Program FAP) services are provided to soldiers and family members: a. Family Violence Awareness (1 ?-2 hour class) annual Unit Troop Education briefings as required by AR 608-18 b. Day 1 FAP briefings (30 minutes) for all in-processing soldiers, conducted daily at Welcome Center c. Domestic Violence Awareness Workshop (DVAW), monthly 3-hour class conducted for soldiers and spouses referred by EACH Social Work Service as part of treatment plan in domestic violence cases d. Unit Prevention Leaders quarterly FAP Overview one-hour training e. Reunion and Reintegration trainings/briefings as requested for redeploying soldiers and family members, ranging from 2-3 hours to one-day Mini PREPs (see g. below) f. Identification & Reporting of Child Abuse/Neglect, 2-hour class conducted monthly at CYS to ensure Center-based caregivers and Family Child Care providers receive annual training per AR 608-10 g. PREP (Prevention & Relationship Enhancement Program), 2-day couples communication classes once/quarter h. A Couples Journey, 2-hour couples class once /month in the evening i. Boot Camp for New Dads, series of three one-hour classes per month j. Nurturing PEACE  3 iterations per year of twelve-week program (consisting of 3-hour weekly sessions), facilitation of adult portion (i.e. parents of children who have witnessed domestic violence) k. Cooperative Parenting & Divorce, four one-hour classes/month or by appointment 2. Contractor will maintain lesson plans and class rosters of attendance/participation and will furnish to FAP Manager/COR by 5th working day of month for input into the ACS Management Report and for ACS Accreditation Standards documentation. Participant numbers for Unit Troop Education and Reunion/Reintegration classes will be furnished to FAPM/COR the following Monday for inclusion in the weekly SITREP. 3. Contractor will maintain record of all Unit Troop Education and participate in the annual Command Inspection Program for all Fort Carson brigades. 4.. Contractor will ensure that evaluation forms are completed by participants at the conclusion of each class. A positive facilitator rating of 85% or higher will be maintained.
