- Contractor shall provide all necessary labor, supervision, materials, supplies and equipment necessary for the inscription of government-owned granite niche covers for Ohio Western Reserve National Cemetery, Rittman, OH. Contract resulting from this solicitation will cover base period of date of award through September 30, 2005, with three 1-year options to extend the term of the contract through September 30, 2008. Inscription work will not be performed at the cemetery. Contractor will be responsible for: pick up; packaging sufficient to prevent damage during shipment and handling; inscribing; and returning niche covers to cemetery. This acquisition is 100 percent set-aside for small business concerns. North American Industrial Classification System (NAICS) Code 561990 and a size standard of $6 M are applicable. All responsible small business sources may submit a quotation. Request for a hard copy of the solicitation must be in writing to Mr. Mark Sanderson, contract Specialist, and accompanied by a check or money order in the amount of Twenty-Five Dollars ($25.00) (non-refundable), payable to "U.S. Department of Veterans Affairs." Please reference Solicitation Number RFQ 786-101-05 on remittances. No telephone requests will be accepted. Hard copy solicitations will be issued via the ordinary mails on or about 04 April 2005. The solicitation may also be obtained FREE OF CHARGE, from VA's Business Opportunities System via the internet at the following URL address on or about 04 April 2005: http://www.bos.oamm.va.gov/solicitation?number=786-101-05.
