- NOTICE OF INTENT TO BUNDLE THIS ANNOUNCEMENT REPLACES ANNOUNCEMENT RFP 797-NC-05-0021 IN ITS ENTIRETY The VA National Acquisition Center will issue Request for Proposal (RFP) 797-NC-05-0025 for the Department of Veterans Affairs (VA) Department of Defense (DOD) Indian Health Service (IHS) and the Bureau of Prison (BOP) requirements for Lactulose Syrup. The following line items are Set-Aside for Small Business only: 1. Lactulose 10gm/15ml Syrup package size 240ml estimated usage-140022 bottles 2. Lactulose 10gm/15ml Syrup package size 480ml estimated usage-251553 bottles 3. Lactulose 10gm/15ml Syrup package size 960ml estimated usage-43502 bottles. The following line items are Unrestricted: 4. Lactulose 10gm/15ml Syrup package size 15ml UD estimated usage-52378 unit doses. 5. Lactulose 10gm/15ml Syrup package size 30ml UD estimated usage- 25489 unit doses. The estimated quantities listed above are for VA DOD BOP and IHS. The contract items will be distributed through the VA and DOD Pharmaceutical Prime Vendors (PPV). Offerors must provide bar code labeling which includes at a minimum the NDC number. Glass bottles are not acceptable. If the offeror is not a manufacturer a signed agreement or letter of commitment between the offeror and the manufacturer must accompany the offer and must remain in effect for the duration of the contract period of performance including four option years. The solicitation will be available for downloading tomorrow 3/17/05 from the VA Office of Acquisition and Material Management?s (O&AMM) internet web site http://www.bos.oamm.va.gov/cgi-bin/WebObjects/BosPublic.woa . Offers will be due no later than 4:00 pm Central Standard Time on April 6 2005.
