- RFQ 786?88-05 Furnish & Deliver Thermoplastic/Thermosetting Graveliners Department of Veterans Affairs National Cemeteries The National Cemetery Administration (NCA) has a requirement for thermoplastic / thermosetting graveliners, delivered F.O.B. Destination within Consignee?s Premises to VA national cemeteries located throughout the 48 contiguous United States, Alaska, Hawaii and Puerto Rico, for use in the interment of casketed remains. Contract covers base period April 1, 2005, or date of contract award, whichever is later, through December 31, 2005. This is a small business set- aside. All responsible small business sources may submit a quote. Requests for a hard copy of the solicitation must be in writing to Mrs. Michelle Cruz, Purchasing Agent, and accompanied by a check or money order in the amount of Twenty-Five Dollars ($25.00) (non-refundable), payable to ?U.S. Department of Veterans Affairs.? Please reference Solicitation Number RFQ 786-88-05 on remittances. No telephone requests will be accepted. Hard copy solicitations will be issued via the ordinary mails on or about March 11, 2005. The solicitation may also be obtained FREE OF CHARGE, at the following URL on or about March 11, 2005. Any amendments to the solicitation will also be available at the URL address. Bidders are advised that they are responsible for obtaining amendments. Numbered Note One is applicable.
