- The Uniformed Services University, the DOD Medical School, is amending the synopsis ?Telephone Survey? Reference PMB5021 originally posted on 2/11/05, to include the following information. Payments may be issued in up to three installments. The first payment may be up to 30% of the contract price and paid after the first month. The second payment may be up to 30% of the contract price and paid after the second month. The third payment will be the remaining amount of the contract price and will be paid after delivery and acceptance of the completed 500 surveys. All contractors must be registered on ccr.gov to receive payment. We can pay via Electronic Funds Transfer (Net30 after receipt of invoice) or a Government Purchase Card. The time period for interviewing has been extended to 9PM. Calls may take place between 9AM and 9PM. The due date for all quotes remains the same and are due no later than 5:00PM EST on Monday, February 28, 2005.
