- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitaiton; quotes are being requested and a written solicitation will not be issed. Requirement reference number : F88MED 4355 0300 is issued as a request for offers, utilizing Simplified Acquisition Procedures in FAR Part 13. The North American Industry Classification System Code (NAICS) is 621399. This acquisition is unrestricted. Under new FAR Guidance and in order to receive this award you must be registered in the Central Contractor?s Regristration data base at http://www.ccr.gov. The contractor shall provide Healthcare services ? namely: Medical Services Technician for Flight Surgeon (4N) required for Government beneficiaries as listed in the Statement of Work; which is incorporated as part of this soliciations and can be requested from ann.robinson@lakenheath.af.mil. The performance period is for one year and seven months. The Base Year is from 1 March 2005 to 30 September 2005, and the Option Year is from 1 October 2005 to 30 September 2006. The Exercise of the Base Year will be at the time of contract award. Funding of the Option Period will be exercised at the time the Option period comes around and is entered into contingent upon funds being made availably for that option year. The U.S. Government will enter into negotiations with the offeror whose offer conforms to the solicitation and provides the overall best qualifications.
