- Contractor shall provide all labor, equipment, material, and supplies necessary for the inscription of decedent data on granite headstones and markers for Southern Wisconsin Veterans Memorial Cemetery, 21731 Spring Street, Union Grove, Wisconsin 53182. Contract resulting from this solicitation will include one base year and up to three 1-year options. Work will include on-site inscriptions on existing preset upright headstones, reverse inscriptions on preset upright headstones, and inscriptions on government-supplied blank flat markers. The contractor shall be required to store blank flat markers in a segregated area, and deliver inscribed flat markers f.o.b. destination. All work shall be performed in accordance with the specifications, drawings, terms and conditions of the contract. This acquisition is 100% set aside for small businesses. The NAICS Code is 561990. Size standard is $6 million. Requests for a hard copy of this solicitation must be in writing. No telephone requests will be accepted. Requests should be addressed to Cheryl Harding, Contract Specialist, U.S. Department of Veterans Affairs, National Cemetery Administration, Centralized Contracting Division (41D3A), 5105 Russell Road, Quantico, VA 22134-3903. Requests for hard copy must be accompanied by a cashier's check or money order in the amount of $25.00 (non-refundable), payable to "U.S. Department of Veterans Affairs." Please reference solicitation number RFQ 786-81-05 on remittance. Hard copy solicitations will be available on or about March 4, 2005, via the ordinary mail. Electronic solicitations will be available for download from the URL address on or about March 4, 2005. Any amendments to the solicitation will also be available at the URL address.
