Description
The REACH 2010 program supports community coalitions in designing, implementing, and evaluating community-driven strategies to eliminate health disparities. Each coalition comprises of a community-based organization and three other organizations, of which at least one is either a local or state health department or a university or research organization, federally recognized Indian tribal governments, and non-federally recognized tribes and organizations. The purpose of this contract is to support up to 42, REACH 2010 grantees with certain evaluation tasks. Therefore, the Centers for Disease Control and Prevention (CDC), National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP), contemplates awarding a cost-plus-fixed fee contract to secure the assistance of an experienced contractor in supporting community-based coalitions with operating an existing interactive internet-based system capable of: data entry, data warehousing, and retrieval of actions and accomplishments displayed in graphs and reports; conducting quality control checks; facilitating cross-coalition learning; and providing on-site and on-line technical assistance. The contract includes a base period of 6 months, and 9 option periods of 6 months each. The total base and option periods shall not exceed 5 years. CDC will consider all responsible sources responding to this Request for Proposals (RFP). All questions/concerns about the RFP shall be submitted via e-mail to BBiltz@cdc.gov. The tentative issue date is 03/07/2005, with the proposal due date of approximately 30 days after posting of the RFP. The complete solicitation package will be available and posted at this site for downloading when issued. No telephone requests of the solicitation will be accepted. No hard copy of the solicitation package will be mailed. The associated NAICS code is 541511 and the small business size standard is $21 Million.
