Description
The purpose of this Amendment to the synopsis/solicitation is to specify travel requirements and to identify the basis for award determination. This is a combined synopsis/solicitation for commercial items, per FAR 13.5 utilizing the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes solicitation N63394-05-T-0053. A written solicitation will not be issued. N63394-05-T-0053 is issued as a Request for Quote on an urgent and competitive basis. The North American Industry Classification System (NAICS) is 541611 and the size standard is $26Million. The contract type is Firm-Fixed-Price with a Base Year and three (3) Option Year Periods. The following items and quantities are required: CLIN 0001 Description Quantity Unit of issue Base Year Provide comprehensive review and identify areas 1 Lot for improvement to the Safety and Health Program, and develop the CNO Safety award package. Provide support services for the Safety and Health Program as specified in the Statement of Work. Period of Performance: ARO through 14 February 2006 Labor Category/Labor Hours Safety and Health Specialists 5000 Total Labor Hours 5000 CLIN 0002 Description Quantity Unit of issue Option Year One Provide support services for the Safety and Health 1 Lot Program as specified in the Statement of Work. Period of Performance: 15 February 2006 through 14 February 2007 Including the following: Labor Category/Labor Hours Safety and Health Specialists 4000 Total Labor Hours 4000 CLIN 0003 Description Quantity Unit of issue Option Year Two Provide support services for the Safety and Health 1 Lot Program as specified in the Statement of Work. Period of Performance: 15 February 2007 through 14 February 2008 Including the following: Labor Category/Labor Hours Safety and Health Specialists 4000 Total Labor Hours 4000 CLIN 0004 Description Quantity Unit of issue Option Year Three Provide support services for the Safety and Health 1 Lot Program as specified in the Statement of Work. Period of Performance: 15 February 2008 through 14 February 2009 Including the following: Labor Category/Labor Hours Safety and Health Specialists 4000 Total Labor Hours 4000 Statement of work provided as follows: PERIOD OF PERFORMANCE: One (1) Base Year with Three (3) Option Year Periods REQUIREMENT: NSWC PHD requires support to conduct a comprehensive review of its Safety and Health program performance to identify areas for improvement to achieve the goal of continuous improvement. NSWC PHD requires the contractor to evaluate the performance of NSWC PHD's current program, identifying opportunities for improvement, determining its current state of compliance with OPNAVINST and SECNAVINST requirements and assessing its progress towards establishing its Safety and Health program as a Navy leader in Safety performance. 1. Develop policies and procedures which ensure the Safety and Health of personnel, facilities, and equipment and compliance with Navy, OSHA, and recognized industry standards for Occupational Safety and Health. 2. Develop and implement management control systems and Safety management information systems for maintaining, reporting and creating best practices for Safety and Health program requirements and initiatives. 3. Develop and implement procedures for injury and illness reporting which ensures compliance with all reporting requirements, identification of root causal factors, and abatement of identified hazards. 4. Develop metrics to assess program performance, establish accountability systems and provide real time status reporting of program performance. 5. Facilitate a self assessment process in compliance with the Department of Navy's Process Review & Measurement System (PR&MS). 6. Develop a plan of action and milestones to achieve the DoD's 50% reduction in injury and illness rates. 7. Develop the Chief of Naval Operations (CNO) Safety award package. DELIVERABLES: 1. Facilitation services for conduct of process review and measurement system/self assessment at NSWC PHD and it's remote detachments located in, San Diego, CA, Louisville KY, Virginia Beach, VA, White Sands, NM. Travel Requirements One (1) Person One trip to detachments: 3 Days Louisville, KY 2 Days Virginia Beach, VA 3 Days White Sands, NM 1 Day San Diego, CA 2. Draft report of PR&MS self assessment by 1 July 05. 3. OSH management balanced scorecard (including metrics/dashboard). Contractor will provide monthly starting 1 April 05. 4. Draft report describing NSWC PHD Safety and Health program attributes and achievements by 15 October 05. 5. Draft POA&M for NSWC PHD OSH program development/process improvement by 1 August 2005. 6. Draft Executive level briefing of NSWC PHD program status and accomplishments by 15 August 2005. 7. Final CNO awards package for signature by the NSWC PHD Commanding Officer and submission due three weeks prior to award submission date. Period of performance for this task is one year: February 15 2005 to February 14 2006 EVALUATION CRITERIA: 1. Demonstrate success in submission and winning the CNO Safety Award. 2. Ability to implement and work Safety and Health Programs per OPNAVIST 5100.23 series. REQUIREMENT: Contractor will provide two (2) full time Safety and Health professionals to complete the following work as described below. Work environment: Work environment is primarily sedentary in nature but at times the incumbent may be required to walk, climb, and kneel during field work such as inspections/investigations. Requirement 1. Serves as a Safety and Occupational Health Specialist and works directly for the NSWC PHD Security and Safety Manager, Code 08. 2. The contractor will be a member of a Team who is responsible for the maintenance of a Safe and Healthful workplace for all of NSWC PHD's personnel, facilities, and equipment. 3. Develops and implements policy and procedures which ensure the safety and health of personnel, facilities, and equipment in compliance with Navy, OSHA, and recognized industry standards best practices for OSH. 4. Develops and maintains metrics to assess safety program performance, utilizing the command dashboard/Portal for monthly updates. 5. Contractor must have a sound working knowledge in the Navy's Process Review & Measurement System (PRM&S), and Self Assessment process. 6. Contractor Is responsible for performing a wide range of Safety and Occupational Health related duties such as conducting all building inspections, management of NAVOSH deficiency notices (Form 5100/12's), job hazard analysis, ergonomic surveys, mishap reduction measures, Identification and correction of deficiency abatement actions. 7.Contractor will manage the Energy Control/Lockout-Tagout program requirements and continually promotes Safety awareness. 8.Contractor will be directly involved with our Team to elevate our command Safety program to world class status. 9.Contractor will participate in the Command's Safety Committee and Safety Council as directed by the manager. 10. The contractor must have a sound working knowledge of the Navy's Occupational Safety and Health Program Manual, OPNAVINST 5100.23 series and various other Safety and Health related regulations/instructions such as OSHA CFR's, Fire and Life Safety, etc. Infrequent travel may be required. 11. The contractor must have attended and successfully gained certification in the basic core Safety courses or equivalent training as determined by the Safety manager per OPNAVINST 5100.23 series, chapter 9: Introduction to NAVOSH Ashore (A-493-0050), General Industry Standards (A-493-0061), Electrical Safety Standards (A-493-0033), Introduction to Hazardous Materials Ashore (A-493-0031), and Introduction to Industrial Hygiene (A-493-0035) and CPR. EVALUATION CRITERIA: 3. Demonstrate knowledge in the various programs required per OPNAV 5100.23 series, and various other Safety and Health related regulations/instructions such as OSHA, CFR?s Fire and Life Safety, etc. 4. Demonstrate required training as directed in OPNAV 5100.23. The award determination of the contract will be based on ?Technically Acceptable Low Price?. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-26 and 27 Published 1 January 2005, effective through 1 January 2005. The offeror must provide the information contained in 52.212-1 instruction to Offeror-Commercial Items, 252.212-7000 Offeror Representative and Certification-Commercial Items and must include a completed of 52.212-3 Alternate 1 Offeror Representatives and Certifications and 252.225-7000 Buy American Act-Balance of Payments Program Certificate with their offer. 52.212-2 Evaluation - Commercial Items, (a) The Government will award a firm-fixed-price contract resulting from this solicitation on a competitive basis. Offerors are to include a completed copy of the provision at 52.213-3, Offeror Representations and Certifications- Commercial Items, with its offer. Clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. Clause at 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders-Commercial Items, applies to this acquisition.
