- 65- Medical Supplies-Potential Sources Sought Military Treatment Facilities (Army, Navy, Air Force, Marine Corps, Coast Guard) TRICARE Pacific Region 12 Patient Assist Device-Walkers Point of Contact: Susan Hwang, Team Leader/Logistician, Tri-Service Regional Business Office, TRICARE Pacific Region 12, Honolulu, Hawaii. Telephone:(808) 433-3686. Potential sources sought to participate in the regional standardization of Walkers. TRICARE Pacific Region 12, a military integrated delivery network (IDN), comprised of Army, Navy, Air Force, Marine Corps and Coast Guard medical treatment facilities in Hawaii, Guam, Korea, Japan and Okinawa are in the process of standardizing Walkers. If your company holds or is in the process of obtaining a Distribution and Pricing Agreement (DAPA) with the Defense Supply Center Philadelphia (DSCP) for Walkers, your company is eligible to participate in the standardization process. This is in pursuant to the DAPA clause by which DAPA holders agree to the standardization process. Please respond by e-mail: susan.hwang@amedd.army.mil and provide your (1) company name, (2) address, (3) point of contact, (4) telephone number, (5) fax number, (6) e-mail address, and (7) answers to the below listed questions no later than March 7, 2005. Also, please send your product literature on Walkers by March 7, 2005 to: Susan Hwang, Tricare Regional Business Office, TPLA MCHK-MM, 1 Jarrett White Road, Tripler AMC, Hawaii 96859-5000. For questions, please call (808) 433-3686. 1. Does your company have a DAPA ? 2. Do you have a distribution agreement with our Prime Vendor Cardinal? 3. What is the brand name of your product? 4. List the different styles that you carry for this product line. 5. Are your products latex free? 6. What is your return goods policy? 7. What is your history of backorders or recalls? 8. Under what standards are your products manufactured? 9. Who is the product representative for our region? Provide name, phone and e-mail information. 10. Do you have an 800-customer service number? 11. Can you provide on-site training and education to all of the facilities in our region (Hawaii, Guam, Okinawa, Japan and Korea) ? 12. What types of education resources and tools for clinical training do you offer for this product line? 13. Is you product safety tested and approved as a walker? 14. Do you have both collapsible sides and fixed sided devices? 15. Are your walkers available in the following configurations: a. All four legs are covered with a durable, grooved tip b. Either the two front or the two back legs can be configured with a wheel(s) c. All four legs can be configured with wheels 16. Is the hand grip area covered with a durable, grooved material which enhances the patients grip? 17. Is the height adjustment accomplished by telescoping feature for all four legs? 18. Can you provide additional replacement parts such as hand grips and wheels?
