Description
The previous combined synopsis/solicitation shall be replaced by the following: Note the due date has also been changed from 2/24/05 to 2/11/05. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested and a written solicitation will not be issued. The simplified acquisition procedures of FAR Part 13 will be used in conjunction with the commercial item procedures in FAR Part 12. The solicitation is being issued as a Request for Proposal (RFP). The solicitation number is RFP is 246-05-01883. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-27. The NAICS is 339111 and the small business size is 500 employees. Items will be purchased and delivery will be made to the Department of Veterans Affairs Medical Center, 1970 Roanoke Blvd, Salem, Virginia 24153. Offeror?s proposals are hereby being requested in response to the requirements under a firm-fixed price contract and shall be submitted on offeror?s letterhead or bid form and shall contain the name, title, and signature of the person authorized to submit proposals on behalf of the contractor. Proposals are to be provided for the following: thirty-five (35) platform hospital beds with mattresses for each of the beds. These items are to be used on a psychiatry ward in a government hospital setting and as a result must meet all federal regulations regarding hospital safety issues. This includes but is not limited to compliance with the most recent 16 C.F.R. part 1632 which can be accessed at www.cpsc.gov All hardware on these items must be tamper-resistant. All corners must be rounded. The mattress must have welded seams and a fluid penetration resistant, antibacterial cover and core. Contractor shall be responsible for removal and disposal of 35 used beds. Proposals shall be valid for 30 days from the due date. All responses must include the following: (1) descriptive literature and product number for the item offered proving the product offered meets the above requirements (2) delivery time after receipt of order (3) cost. The following provisions and clauses apply to this solicitation: 52.212-1, Instructions to Offeror-Commercial Items, 52.212-2 Evaluation ? Commercial Items, 52.212-4, Contract Terms and Conditions-Commercial Items, 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, 52.233-2 Service of Protest, 852.233-70 Protest Content, 852-233-71 Alternative Protest Procedure. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.The following factors shall be used to evaluate offers: technical, past performance and price/price related factors. Technical and past performance, when combined, are more important than price. Proposals, including the pricing information are to be submitted on company letterhead and include the information required in provisions 52.212-1 and a completed copy of the provisions 52.212-3, Offeror Representations and Certifications-Commercial Items. In accordance with FAR 52.204-7 award will be made only to a contractor/company that is registered in the Central Contractor Registration (CCR). Electronic copies of the referenced provisions and clauses are available on the internet at www.arnet.gov and www.va.gov/oa&mm/vaar/ or fax a request to 757-728-3132. Proposals and all required information must be received by noon local time February 11, 2005. Quotations shall be submitted to Barbara Simmons, Contracting Officer (90C), VA Medical Center, Hampton, Va. 23667. No faxed offers will be accepted. Solicitation number RFP 246-05-01883 shall be annotated on the documents submitted for identification purposes. Only offers prepared in accordance with this solicitation shall be considered.
