Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) This solicitation is issued as a request for quotation (RFQ) using simplified acquisition procedures and shall be awarded on an all or none basis. (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular Fac 2001-26 & 27, Effective 01 Jan 2005. (iv) This solicitation is a 100% Small Business Set-Aside. The associated NAICS code is 311991. The small business size standard is 500 employees. (v) Offerors shall structure their offer using the following items: LINE ITEM 0001  Box Lunch - 4Mar05, 850 EA $_______, $________, LINE ITEM 0002  Box Lunch - 5Mar05, 850 EA $_______, $________, LINE ITEM 0003  Box Lunch - 17Mar05, 850 EA $______ _, $________, LINE ITEM 0004  Box Lunch - 18Mar05, 850 EA $_______, $________, LINE ITEM 0005  Box Lunch - 19Mar05, 850 EA $_______, $________, In addition to the line item schedule, offers shall include a sample meal submitted with offer. This ev aluation is subjective in nature but will include such values as freshness and name brand items known for popularity and quality among consumers. Offerors may make multiple offers if desired. Sample box lunches shall be delivered with offerors price prop osal to the base contracting office between the hours of 11 AM and 1 PM on 9 February 2005. Contact the Contracting Officer, MSgt James Cunningham at (804) 236-6355 for an appointment time and instructions on base access. Offeror agrees, by submission of a price proposal(s) and sample(s), that the sample(s) provided are submitted at no cost to the government for the sole purpose of evaluating potential awardee box lunch contents. Offer certifies that contents in submitted samples shall be the same as those provided in the event you are awarded a contract. (vi) Description: The contractor shall furnish all labor, materials, supervision, transportation and facilities necessary to provide fresh box meals in accordance with the following; 0001 Box Lunch - 4 Mar 05 Box Lunch consisting of 1 Ham & Cheese Hogie Sandwich; 1ea zip lock bag sealed containing lettuce tomato and pickle; 1 can soda (assorted kinds); 1 cookie; 1 apple or orange; 1 Bag of chips; 1 Boiled egg; 1 box raisons; 1ea 1/2 size candy bar; and individu al pack of mustard & mayo salt pepper plastic knife and napkin Contractor shall deliver the box lunches to Va Air Guard on 4 March 05 NLT 3 PM. 0002 Box Lunch - 5 Mar 05 Box Lunch consisting of 1 Roast Beef Hogie Sandwich; 1ea zip lock bag sealed containing lettuce tomato and pickle; 1 can soda (assorted kinds); 1 cookie; 1 apple or orange; 1 Bag of chips; 1 Boiled egg; 1 box raisons; 1ea 1/2 size candy bar; and individual pack of mustard & mayo salt pepper plastic knife and napkin. Contractor shall deliver the box lunches to Va Air Guard on 5 March 05 NLT 3 PM. 0003 Box Lunch - 17 Mar 05 Box Lunch consisting of 1 Ham & Cheese Hogie Sandwich; 1ea zip lock bag sealed containing lettuce tomato and pickle; 1 can soda (assorted kinds); 1 cookie; 1 apple or orange; 1 Bag of chips; 1 Boiled egg; 1 box raisons; 1ea 1/2 size candy bar; and individu al pack of mustard & mayo salt pepper plastic knife and napkin. Contractor shall deliver the box lunches to Va Air Guard on 17 March 05 NLT 3 PM. 0004 Box Lunch - 18 Mar 05 Box Lunch consisting of 1 Turkey & Cheese Hogie Sandwich; 1ea zip lock bag sealed containing lettuce tomato and pickle; 1 can soda (assorted kinds); 1 cookie; 1 apple or orange; 1 Bag of chips; 1 Boiled egg; 1 box raisons; 1ea 1/2 size candy bar; and indiv idual pack of mustard & mayo salt pepper plastic knife and napkin. Contractor sh all deliver the box lunches to Va Air Guard on 18 March 05 NLT 3 PM. 0005 Box Lunch - 19 Mar 05 Box Lunch consisting of 1 Roast Beef Hogie Sandwich; 1ea zip lock bag sealed containing lettuce tomato and pickle; 1 can soda (assorted kinds); 1 cookie; 1 apple or orange; 1 Bag of chips; 1 Boiled egg; 1 box raisons; 1ea 1/2 size candy bar; and individua l pack of mustard & mayo salt pepper plastic knife and napkin. Contractor shall deliver the box lunches to Va Air Guard on 19 March 05 NLT 3 PM. (vii) Delivery shall be FOB Destination. (viii) The provision at 52.212-1, Instructions to Offerors--Commercial, applies to this acquisition. (ix) Basis of Award. 52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i)must pass our public health evaluation in order to be awarded a contract (ii) price; (iii) past performance; (iv) evaluation of the quality of a sample meal submitted with offer. This evaluation is subjective in nature but will include such values as f reshness and name brand items known for popularity and quality among consumers. Offerors may make multiple offers if dersired. Before the of fer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
