Description
Request for Quote (RFQ)200-005-5-NE is hereby corrected to read: The Bureau of Prisons is seeking quotations from sources having the ability to provide community transitional drug abuse treatment and mental health services to male and female Federal inmates in the custody of the Federal Bureau of Prisons. The Government anticipates up to three awards for services within three geographically-defined districts in Brooklyn and Manhattan, New York. This is a firm-fixed indefinite-delivery, indefinite-quantity type contract. The geographically-defined areas are 1)Brooklyn, New York (no farther west than Clinton Street, no farther south than Pacific Street, no farther north than Tillary Street and no farther east than Flatbush Avenue); 2) Manhattan, New York (on or between 9th and 7th Avenues, but no farther south than 34th Street and no farther north than 48th Street); 3)Manhattan, New York (on or between 7th and Park Avenues, but no farther south than 34th Street and no farther north than West 48th Street). The Government reserves the right to make single or multiple awards. Performance period under this contract will be for a one year base period to begin on May 1, 2005 with four (4) one year option periods, with service ending (if all option years are renewed) on April 30, 2010. The services and quantities required for this contract are as follows: Base Period - Intake Assessment (1 per inmate) 40 minimum/95 maximum; Individual Counseling (16 units per inmate) 640 minimum/1,520 maximum; Group Counseling ( 67 units per inmate) 2,680 minimum/6,365 maximum; Mental Health Intake Assessment and Report (1 per inmate) 2 minimum/10 maximum; Medication Monitoring (2 units per inmate) 4 minimum/20 maximum. Option Year One - Intake Assessment (1 per inmate) 42 minimum/100 maximum; Individual Counseling (16 units per inmate) 672 minimum/1,600 maximum; Group Counseling (67 units per inmate) 2,814 minimum/6,700 maximum; Mental Health Intake Assessment and Report (1 per inmate) 2 minimum/12 maximum; Medication Monitoring (2 units per inmate) 4 minimum/24 maximum; Option Year Two - Intake Assessment (1 per inmate) 44 minimum/105 maximum; Individual Counseling (16 units per inmate) 704 minimum/1,680 maximum; Group Counseling (67 units per inmate) 2,948 minimum/7,035 maximum; Mental Health Intake Assessment and Report (1 per inmate) 2 minimum/15 maximum; Medication Monitoring (2 units per inmate) 4 minimum/30 maximum. Option Year Three Intake Assessment (1 per inmate) 46 minimum/110 maximum; Individual Counseling (16 units per inmate) 736 minimum/1,760 maximum; Group Counseling (67 units per inmate) 3,082 minimum/7,370 maximum; Mental Health Intake Assessment and Report (1 per inmate) 2 minimum/15 maximum; Medication Monitoring (2 units per inmate) 4 minimum/30 maximum. Option Year Four - Intake Assessment (1 per inmate) 48 minimum/115 maximum; Individual Counseling (16 units per inmate) 768 minimum/1,840 maximum; Group Counseling (67 units per inmate) 3,216 minimum/7,705 maximum; Mental Intake Assessment and Report (1 per inmate) 2 minimum/15 maximum; Medication Monitoring (2 units per inmate) 4 minimum/30 maximum. The services described shall be in accordance with the Federal Bureau of Prisons Transitional Drug Abuse Treatment (TDAT) Statement of Work, dated January 2004, Northeast Region, Revised February 2004. The solicitation will be available on or after February 7, 2005, and will be distributed solely through the General Services Administration's Federal Business Opportunities web site http://www.fedbizopps.gov/. Hard copies of the solicitation document will not be available. The site provides downloading instructions. All future information about this acquisition, including solicitation amendments, will also be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. The estimated closing date is o/a March 7, 2005. All responsible sources may submit a quote which will be considered by this Agency. Faith-Based and Community-Based organizations have the right to submit offers/bids/quotes equally with other organizations for contracts for which they are eligible.
