Description
Darnall Army Community Hospital Podiatry Clinic, Fort Hood, Texas has a requirement for the rental of a Dornier Epos Ultra Extracorporeal Shock Wave Therapy (ESWT) unit to include transportation and unit operator (i.e., Certified Radiology Technician ). The Epos Ultra unit is manufactured by Dornier MedTech (Brand Name). Federal Acquisition Regulation (FAR) Clause 52.211-6, Brand Name or equal, applies to this requirement. An or equal ESWT unit offered in lieu of the Brand Name Dornier ESWT shall meet the following salient characteristics to be considered: TECHNICAL DATA MAIN MODULE: Footprint (l x w): 37 x 24; Height: 43; Weight: 275 lbs.; Power Supply: 100  125/200-240 V 1000 VA; +/-10% 50/60 Hz; Shock Source: Dornier EMSE 0-80. SHOCK WAVE DATA: Principle: Electromagnetic; Aperture: 5.5; Aperture Angle: 82 degrees; Energy Flux Density ED (mJ/mm2): 0.13  1.0; Effective Energy E(12 mm) (mJ): 3.8  27.3; Focal Pressure (MPa): 7.4  55.7; Focal Dimension: 0.3  1.7; Penetration D epth: 3; Shocks/Minute: 60  240. The ESWT unit and operator will be transported to the Podiatry Clinic. The operator will activate the ESWT for the Podiatrist wh o will treat patients afflicted with Plantar Fasciitis. This medical procedure at the Podiatry Clinic averages about ten (10) per month, but may vary more or less depending on case load. The contractor will be provided a seven (7) day advanced notificati on of scheduled medical procedures at the Podiatry Clinic. The period of performance is from date of award in February 2005 through January 31, 2006 with two option years. This procurement is being conducted as a commercial item acquisition in accordance with FAR part 12. All responsible sources may submit an offer, which will be considered.
