- ANY REQUESTS MUST BE WITH YOUR COMPANY NAME, ADDRESS, TELEPHONE AND FAX NUMBER, PLEASE NO EMAILS UNABLE TO ACCESS IT. The Secretary of the Navy Council of Review Boards has a requirement to reserve a hotel in New Orleans LA in the 300 block on Bourbon Street with the following: Conference room plus four (4) additional break rooms to accommodate approximately 85 personnel for the period of 9 May 2005 through 13 May 2005, for a Annual Physical Evaluation Board Conference. The hotel must be able to provide rooms for the personnel while attending the conference. These rooms shall be together (same floor or wing), the rooms shall be paid by the individuals please list the cost for each roomalso on your proposal. The following shall be required: Clin 0001 ? Rental of Office/Storage, General Session (classroom) and Meeting Rooms. 5 Days. Clin 0002 ? Wireless Microphone. 4 Days. Clin 0003 ? 8 foot by 8 foot screen. 4 Days. Clin 0004 ? Office phone lines (plus toll charges). 5 Days. Hotel must have a computer room, transportation to and from airport, continental breakfast for Tuesday through Friday, mid Morning Break for Tuesday through Thursday with refreshments and snack, Afternoon break for Tuesday through Thursday with refreshments and snack, and Ice breaker hot and cold finger food and non alcoholic drinks.
