Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Please refer to Request for Quote Number: W912CZ-05-T-0015. The provisions and clauses incorporated are those in effect through Federal Acquisition Circular (FAC) 2001-26. The requireme nt is an open market solicitation for SIC Code: 5047; NAICS Code: 423450; Size Standard: 100 employees. Closing date is 28 January 2005 at 1:00 pm Alaska time. Product requested: Six self-contained, portable (weighing no more than 60 pounds), stand-alo ne, medical training mannequins/devices of which the salient characteristics are as follows - intubation (oral and nasal) with obstructed airway; orpharyngeal and nasopharyngeal airway insertion; fiberoptic and lightweight intubation; retrograde intubation ; needle and surgical cricothyrotomy; stomach auscultation; chest decompression (mid-clavicular and mid-axillary); articulating blood pressure training arm; palpable bilateral carotid pulse; peripheral IV training; brachial and radial pulses; chest compres sion; synchronized variable rate, rhythm, abnormalities and duration of simulation arrhythmias; programmable scenario-based algorithms and waiting rhythms for instructor control; 3-4 lead ECG, pacing, and defibulation capabilities (25-360j); selectable lib rary of heart, lung, and bowel sounds; vocal sounds - computer generated sounds or live feed via microphone; auscultated and palpated blood pressure with brachial and radial pulse control; bilateral carotid, brachial and radial pulses dependent on blood pr essure; initial on-site training, and software upgrade support should be provided with the mannequins and software. FAR and DFARS Clauses and Provisions: 52.212-1, Instruction to Offerors; 52.212-2, Evaluation -- Commercial Items (The Government will award a contract resulting from this solicitation to the responsible offeror whose offer is the best value to the Go vernment. The following factors shall be used to evaluate offers: 1) Technically acceptable, which is approximately twice as important as price; 2) price. Options: The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligated the Government to accept the option(s). A writt en notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party.
