Description
Department of Veterans Affairs Medical Center, 7305 N. Military Trail, West Palm Beach FL 33410-6400 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. The simplified acquisition procedures in FAR Part 13 will be used in conjunction with the commercial item procedures in FAR Part 12. The solicitation is being issued as a Request for Quotation (RFQ). This solicitation number is 548-05-297. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-27. This acquisition is a single award and is unrestricted. A firm fixed price indefinite delivery, indefinite quantity order is anticipated. The North American Industry Classification (NAICS) is 424210 with the size standard of 100 employees or smaller. LINE DESCRIPTION EST. QUANTITY UNIT UNIT TOTAL ITEM PRICE PRICE Base Period 2/3/05 _ 3/17/05 1 F-18 FDG ISOTOPES 70 EA $_______ $_______ (7 weeks at 10 doses per week) Option Period 3/24/05 _ 6/16/05 2 3 Month Option 130 EA $_______ $ _______ (13 weeks at 10 doses per week) The following is required: F-18 FDG Isotopes, to be used for patient exams. The doses will be required one day per week, to be used in the mobile PET/CT unit. The doses shall have a 110 minute half-life and should be calibrated according to patient appointment time. F-18 FDG are the only isotopes to be used. Add on doses are expected within 4 hours from a call by the technologist. Unused doses shall be returned by the vendor pickup along with the used containers and credited by the vendor. Doses are to be delivered in a vehicle by the vendor. Prescription numbers of delivered doses shall be emailed to the Lead Nuclear Medicine point of contact to be provided after award. The contract period shall be from February 3, 2005 through March 17,2005 with one three month option. FOB Delivery to made to VA Medical Center, 7305 North Military Trail, West Palm Beach, FL 33410-6400. The provision at 52.212-1, Instructions to Offerors _ Commercial, applies to this acquisition. Addendum to FAR 52.212-1, Instructions to Offerors- Commercial Items _ Sub-Part 13.5 Test Program applies. FAR 52.212-2, Evaluation- Commercial Items applies. The evaluation criteria will be based upon price and the offeror must be able to meet the requirements listed above. The Government intends to evaluate the information received and award without discussion. Quotes shall be submitted and shall include the information required in FAR 52.212-3, Offeror Representations and Certifications _ Commercial Items (Jan 2005).
