Description
Medical Supplies - Potential Sources Sought Military Hospitals: (Army, Navy, Air Force) Southeast and Southwest Region Patient Care Plastics: Standardization Point of Contact: Cindy Ingrao, Clinical Consultant, Tri-Service Regional Business Office, Southwest Region, San Diego, CA. Telephone: (619) 532-8517. Potential sources sought to participate in the regional standardization of Patient Care Plastics (pitchers, drinking cups, emesis basins, bed pans, wash basins, urinals, sitz baths, etc.). Southeast and Southwest Regions, a military integrated delivery network (IDN), comprised of Army, Navy and Air Force medical treatment centers in Alabama, Mississippi, Tennessee, Georgia, South Carolina, Florida, and California are in the process of standardizing Patient Care Plastics. If your company currently holds, or is in the process of obtaining a Distribution and Pricing Agreement (DAPA) with the Defense Supply Center-Philadelphia (DSCP) Patient Care Plastics, your company is eligible to participate in the standardization process. Please respond by Email: cingrao@nmcsd.med.navy.mil and provide your company name, address, point of contact, telephone, fax and E-mail address and answers to the below listed questions no later than February 11, 2005. For questions, please call (619) 532-8517. 1.Does your company manufacture patient plastics (pitchers, drinking cups, emesis basins, bed pans, wash basins, urinals, sitz baths, etc) and not including specimen containers and collectors? If so, what types of plastics does your company manufacture? 2.Are your products available through the Prime Vendor, Owens & Minor? 3.What is your DAPA number? 4.Under what brand(s) does your company manufacture or distribute your products? 5.Under what standards are your products manufactured? 6.Are your products latex free? 7.Do you use non-toxic, color fast dyes in your products? 8.How durable are your products? 9.Do your products have smooth edges? 10.Are your products sturdy and non-collapsible? 11.Is the use and function of the product easily identifiable? 12.Are numbers and characters on the products readable? 13.Do your products have an accessible area for easy application of the patient ID label? 14.Do your products come in different sizes? If so, what sizes? 15.Do your products come in different colors? If so, what colors? 16.Does your company provide customer service 24/7? 17.Is there any history of backorders and/or recalls for this product group?
