- This modification is issued to notify you that the response date should read February 18, 2005. Provide medical services to three Federal Law Enforcement Training Center (FLETC) facilities located at; (1) Glynco, Georgia (main facility); (2) Artesia, New Mexico; and (3) Charleston, South Carolina. The services to be provided under this contract will include but not limited to (i) sick calls; (ii) fitness examinations; (iii) annual physicals; (iv) record keeping; and (v) reports. The staffing will require at a minimum one doctor, LPNs and possibly a Physician Assistant at each of the three designated sites. The staffing and student levels at each location vary and will be included in the solicitation uniform issue and laundry support services. NAICS Code 621111 is used for this project. Contractor will be required provide all management, personnel, supervision, materials, supplies required to and have in-depth knowledge of management of medical services contracts. Contract will be awarded with a base period (April 1, 2005 through September 30, 2005) and four additional one-year option periods. The awarded contract will be a firm fixed price type contract.
