- Services Nonpersonal: Provide all labor, materials, equipment and transportation necessary to provide LICENSED PHYSICAL THERAPIST SERVICES located at MacDill AFB, FL, in accordance with the attached Performance Work Statement. The period of performance is: 01 April 05 through 30 Sep 05 with four additional option years, pending availability of funds. This acquisition is 100% set-aside for small business concerns and will be acquired under procedures set forth under FAR Part 13.5. The applicable NAICS code is 621340 and the size standard is $6,000,000.00. All responsible small businesses, capable of supplying the equipment and performing this service are encouraged to submit an offer, which will be considered by this office. Offers from large businesses will not be considered. The government intends to make award in the form of a firm-fixed price contract. The anticipated issue date of the solicitation is on/around 4 Jan 05 and will be available for download at www.fedbizopps.gov. The solicitation close date is scheduled for on/around 18 Jan 05. All offerors are responsible for registry in the Central Contractor Registration (CCR) prior to being eligible for any government award. This may be accomplished at www.ccr.gov or by calling 1-888-227-2423. Offerors are reminded that it is their responsibility to check the www.fedbizopps.gov site for any amendments that may be issued to this notice.
