- Contractor to provide Continuous JCAHO Survey Readiness Program for Department of Veterans Affairs, Great Lakes Healthcare System (VISN 12) facilities in accordance with specifications. All contract services to be completed during the period of March 1, 2005 through September 30, 2005. This procurement is 100% set-aside for Small Business. Quotations received will be evaluated on technical capability, price and past performance. Solicitation documents will be available electronically January 3, 2005, with quotations due January 21, 2005. NAICS is 611710 with a small business size standard of $6.0 million in revenues. Offerors are reminded to complete all sections of the solicitation, including the Offeror Representations and Certifications-Commercial Items. Requests for hard copy solicitation must be in writing to the Contracting Officer at the Great Lakes Acquisition Center and accompanied by a check for $5.00 made out to the Department of Veterans Affairs. No telephone requests will be accepted. Offerors are advised that they are responsible for obtaining amendments.
