- This is a commercial item solicitation using FAR Part 12 and FAR Part 15 for providing community based primary medical care services to eligible veteran beneficiaries for the VA Salt Lake City Health Care System in the counties surrounding and counties of Jackson, Lincoln, Sublette, and Sweetwater, Wyoming. The services shall be all inclusive; i.e., health care provider shall furnish all supplies, etc. Facility shall (1) meet all applicable state and federal requirements or standards; (2) meet standards set forth in the JCAHO Accreditation Manual for Hospitals or current Rural Health Designation. Contract will be a total of five years which will include base year and four options to extend. Contract period shall be May 1, 2005 through April 30, 2006. Issue date on or about December 23, 2004, closing on or about February 11, 2005. This acquisition is subject to the availibility of funds.
