- NA (1)This is a request for commercial services prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6. Proposals are being requested and written solicitations will be issued. (2) This solicitation is issued as RFQ 573-001-05. (3) The solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular (FAC) 2001-25. (4) This solicitation is advertised as a Small Business Set Aside with NAICS: 446130 with a size standard of $5 million. The procurement shall be conducted under FAR 13.5 Test Program, simplified acquisition procedures, and a combination of FAR 12, 13, and 15. The Contractor shall furnish all necessary supervision and qualified competent personnel to provide eyeglasses and dispensing of eyeglasses for the Department of Veterans Affairs, North Florida South Georgia Health System. Services shall be performed at the VA Medical Center, Gainesville, VA Medical Center, Lake City, and the VA Satellite Outpatient Clinic, Tallahassee, FL. The Contractor shall comply with JCAHO requirements and maintain competence assessments for the contract personnel selected to perform services for this contract. The VA intends to award single or multiple award contracts. The Government intends to make an award based on the best value to the Government by performing a price/technical tradeoff analysis. The contract base period will be for one period effective from date of award through September 30, 2005. Total duration of the contract will not exceed 4 years. The solicitation will be available for downloading by January 14, 2005. The closing date for receipt of all proposals is February 18, 2005 at 3:00pm local time.
