- Description
- The Centers for Disease Control and Prevention intends to purchase Sulfamethoxazole ring (13C6) 100mg, 1 each and Ciproflaxin (13C3) 100mg, 1 each. The Government intends to evaluate offers and award a contract without discusssions with offerors providing the requested product. Therefore, the initial offer should contain the offeror?s best terms from a technical and price standpoint. The North American Industry Classification (NAICS) Code is 421450 for this requirement. This requirement is not a small business set-aside. Interested vendors capable of furnishing the Government with the products in this synopsis should submit a quotation to the below address. The quotation shall include the itemized price, prompt payment terms, the F.O.B. point (Destination or Origin), Taxpayer Identification Number (TIN), Dun & Bradstreet Number (DUNs). The quotation must reference Solicitation Number 2005-Q-16640. Quotations will be due five (5) calendar days from the publication date of this synopsis or December 14, 2004 by 3:00 pm EST, whichever is later. All responsible sources may submit a quotation, which if timely received, shall be considered by the agency. Quotations may be submitted in writing to the Centers for Disease Control and Prevention, 2920 Brandywine Road, Atlanta, GA 30341, ATTN: Tommy Dorsey. E-mail quotations to tjd6@cdc.gov. This action is pursuant to USC 253(1) and under the authority of FAR 6.302-1. The requirement will be procured as a commercial item in accordance with FAR 12.102 ? under the test program. THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDEDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. The following provisions apply to this acquisition: 52.212-1, Instructions to Offerors ? Commercial Items (MAR 2000) and 52.212-2, Evaluation ? Commercial Items (JAN 1999); the evaluation criteria stated in paragraph (a) of this provision are as follows: (1) technical capability of the to furnish the products stated herein, and (2) price. The clause 52.212-4, Contract Terms & Conditions ? Commercial Items (MAY 1999) applies to this acquisition. The following FAR clause is hereby added: 52.247-34, F.o.b Destination. The clause 52.212-5, Contract Terms & Conditions Required to Inplement Statutes or Executive Orders ? Commercial Items, applies to this acquisition. The following clauses listed in paragraph (b) of 52.212-5 apply to this acquistion: (1), (3), (5), (7), (9), (11), (12), (14), (15), (17), (18), (20) and (24).
