Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information in this notice. This announcement constitutes the only solicitation; a quote is being requested and a written solicitation will not be issued. The NAICS code is 541990 with a size standard of $6 Million. This Request for Quotation (RFQ) Number is RFQ 101-13-05 is for Quality Assurance Services for Pathologists and Cytotechnologists. The period of performance is one year from date of award with four (4) one-year option periods. The Government contemplates award of a requirements contract, as defined under FAR Subpart 16.503, with an estimated quantity resulting from this Request for Quote. The primary place of performance shall be performed on a national basis for various VA facilities. Exact locations will be provided upon contract award. The Pathology and Medical Laboratory Service is seeking continuous quality improvement (CQI) in cytopathology as stated in Pathology and laboratory Medicine Services (P&LMS) Handbook 1106.1. Enrollment in external quality improvement programs can best fulfill this requirement. The American Society of Clinical Pathologists (ASCP) Checkpath Program is the mandatory external CQI source that has been used in the past to fulfill this requirement. This CQI program will be contracted and monitored by the National Director, P&LMS. The Department of Veterans Affairs (VA) requires a contract to furnish a quality assurance program in cytopathology to pathologists and cytotechnologists at approximately 125 VA Medical Centers throughout the Continental United Stated. At these 125 Medical Centers there are approximately 377 cytotechnologists and pathologists participating in ?CQI?. This program is designed to measure and to assist in the delivery of quality care by physicians. This program will provide self and peer evaluation of pathologists through the diagnosis of individual cases. This program puts the emphasis on those factors shown to generate the greatest potential of risk to the patient and those most important to patient care. This requirement will be awarded using the lowest price technically acceptable source selection process as described in FAR Subpart 15.101-2(b)(1). Award will be made on the basis of the lowest evaluated price of quotes meeting or exceeding the acceptability standards for non-cost factors. The standard is that the vendor must be able to meet all tasks as described under 2.0 of the Statement of Work. Also, the vendor supplying the CQI surveys must have ?Deemed Status? by the Centers for Medicare and Medicaid Services (CMS) and must be approved by both the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and the College of American Pathologists (CAP) to provide CQI surveys to facilities they accredit. If these standards are not met, the vendor will ineligible for award of the contract. Attached to this combined synopsis/solicitation is the Statement of Work, the deliverable price schedule, and Offeror Certifications to for the vendor to fill. As this contract will be a requirements contract, the quantity is an estimate and the actual amounts may be more or less. The Contractor shall include a fully executed copy of the Federal Acquisition Regulation (FAR) provision 52.212-3, Offeror Representations and Certifications-Commercial Items (Jan 2004).
