- This is a combined synopsis/solicitation for commercial items and services prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. Solicitation number F73MED43013000 is hereby issued as a Request for Quote. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 01-25. This acquisition is a total small business set-aside and is under North American Industry Classification Standard (NAICS) code 561421, with a size standard of $6 Million. Contractor will provide a 24 Hour Nurse Advise Line in accordance with the attached statement of work. Award shall be made only to contractors who have registered with the Central Contractor Registration at: www.ccr.gov/. Contractor shall submit their pricing on the attached Pricing Schedule, which is available for download. The contractor shall meet the requirements of the Statement of Work, which is available for download. Period of performance is for a base year plus 4 option years. The basic year performance period is 01 Dec 2004 through 30 Nov 2005. The following clauses and provisions are incorporated and are to remain in full force in any resultant purchase order: FAR 52.212-1 (OCT 2004), Instructions to Offerors- Commercial Items is incorporated by reference. It is amended to read: Submit signed and dated offer to 97 CONS/LGCA, 303 J Ave., Bldg 302, Altus AFB OK 73523-5132 at or before 1200 (12:00 P.M.) CST, Monday November 22, 2004. Submit signed and dated quotations on company letterhead or the attached Bid Schedule Document. Quotes must contain pricing for all line items for the Base Year and the four Option Years.
