Description

Provide Community Based Home Care, Skilled, Homemaker, and Home Health Aide Services for beneficiaries of the VA Salt Lake City Health Care System. Offers will be evaluated from contractors who can provide service in the following areas Garfield County Utah and Kane County Utah. Must be Medicare Certified. Must have licensed RN as Director of Nursing. Must have a licensed RN taking home health orders. Priority will be given to non-profit home care providers. Contract will run for one year from date of award with four one-year options to extend the contract. This solicitation is subject to the availability of funds. Issue date November 16, 2004. Closing date December 9, 2004.
