Description
This is a combined synopsis/solicitation for commercial Non-Personal Service Contract prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation WILL NOT be issued. Solicitation Number RFP-797-FDF9-05-0002 is issued as a Request for Proposal. This solicitation document and incorporated provisions and clauses are those in effect through FAC 01-25. The NAICS Code is 561110 and the small Business Size is $6.0M employees. This action is 100% Set Aside for Small Business. The Air Force Medical Logistics Office/Veterans Affairs Special Services (AFMLO/VASS) Contracting Activity has a requirement for three (3) administrative support personnel for Scott AFB, IL. The administrative support personnel shall coordinate health services management functions and ensure team members receive information in a timely and effective manner. They shall also coordinate specialty care referrals, serve as functional experts in medical record management and maintain health records and their storage and retrieval system. The administrative support personnel shall participate with PCM team members in providing, assessing, and improving a wide variety of customer services/relations as well as assist with the management of all forms of communication between beneficiaries, PCM team members, network/outside providers, and ancillary health care workers. FORMAL EDUCATION. The administrative support personnel shall have a high school diploma, or General Educational Development (GED) equivalency. Completion of a Medical Assistant program is desirable. Training in Windows 2000, Windows NT and Excel is desirable. Complete and maintain Basic Life Support (BLS) Training as required by the MTF. BLS training will be provided by the MTF. EXPERIENCE. The provider shall have the following experience levels: The personnel performing this duty will be knowledgeable in general medical ethics, health records administration, telephone etiquette, office management methods, excellent communications and customer service skills, and strong organizational background and computer operations to include Windows 2000, Windows NT, and spread sheet type applications. Personnel shall understand the importance of deadlines. The following FAR clauses apply to this acquisition. FAR clause 52.212-1, Instructions to offerors ? Commercial Items; FAR Clause 52.217-8 Option to Extend Services; FAR Clause 52.217-9 Option to Extend the Term of the Contract; FAR Clause 52.232-19 Availability of Funds for the Next Fiscal Year; FAR Clause 52.216-1 Type of Contract; and FAR Clause 52.217-4 Evaluation of Options Exercised at Time of Contract Award. VAR Clause 852.203-71 Display of a VA Hotline Poster is also applicable. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certification ?
